Documentation of Patient Contact Hours (included call back hours included in the rate and charged call back hours) Physicians are required to provide supporting notes listed on their timesheet entries to verify Call Back hours (patient contact hours) mentioned above.

Physicians must document in the timesheet the specific reason for the call back and they should have a description such that the note can be validated if an audit report was generated in an electronic health record.
