2. Training and Experience: Must have good communication skills. Previous experience in clinical nutrition preferred. Some computer knowledge necessary.

The Registered Nurse Cardiac Cath Lab RN plays a critical role in providing specialized nursing care to patients undergoing cardiac catheterization and related interventional procedures. This position requires the nurse to collaborate closely with cardiologists, technologists, and other healthcare professionals to ensure patient safety and optimal procedural outcomes. Minimum Qualifications:
- Current and valid Registered Nurse (RN) license in the United States.

DESCRIPTION:
The role of the Care Coordinator is to coordinate services for all patients who are part of the assigned panel, especially those with serious, complex, or chronic health problems or with psychosocial issues. Patient Care Medical Home coordination includes an initiative-taking approach to deliver complete preventive care, and provide advocacy, information and referral services to patients and families to address their medical and psychosocial needs. The Care Coordinator Team Lead will oversee and coordinate these functions.
