ESSENTIAL FUNCTIONS:
· In collaboration with multidisciplinary team, evaluates patient referrals to develop individualized plan of care across the continuum of care

· Collaborates with patients/families/populations to coordinate community services and appropriate resources which meet the individual needs and goals.
· Serves on selected hospital, group practice and community-based committees.
· Identifies need for and participates in research projects relevant to specific patient population.
· Collaborates with Quality Management, Utilization Review, Information Technology Services, and other departments regarding data availability and reports.
· Is accountable for the implementation and communication of standards for patient care delivery appropriate to the specific patient population that meets professional and regulatory standards.
· Coordinates the health care team in development of clinical tools for optimal outcomes and reports findings.
MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
· Current Iowa registered nurse (RN) license.
· Bachelor’s degree in health-related field required or actively pursuing. BSN preferred.
· Demonstrates competency within a specialized area of clinical practice with 4 years experience in that specialty.
· Proof of completion of Mandatory Reporter – Child and Dependent Adult Abuse training within three (3) months of hire.
· Basic Life Support certified within six (6) weeks of hire. Acceptable credentialing bodies and certifications include American Heart Association Basic Life Support for Healthcare Providers.

General Requirements:

Education:

GENERAL SUMMARY:
Provides professional emergency nursing care for acutely ill and injured patients incorporating the nursing process of all age groups including neonates, infants, children, adolescents, adults, and geriatrics. Provides psychological support based on assessed need.
Utilizes multiple teaching methods. Discharge instructions are reviewed and understanding acknowledged by the patient/family.

Supervises Emergency Department Technicians.
Serves as a preceptor for students and employees (i.e. EMS, nursing, new employees).
Works independently using the Nurse Practice Act, hospital policies, and professional judgment. Makes recommendations to improve the Emergency Department’s Standards of Care.
MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
Possession of a current or compact state license as a registered nurse issued/defined by the State of Iowa.
One year of RN experience preferred OR new graduate if the candidate has completed senior nursing preceptorship at Mercy Emergency or previous experience as an emergency department technician or experience in a pre-hospital environment.
Proof of completion of Mandatory Reporter abuse training specific to population served within 3 months of hire.
Basic Life Support (BLS) certified within six (6) weeks of hire. Acceptable credentialing bodies and certifications include American Heart Association Basic Life Support.
Completion of Advanced Cardiovascular Life Support (ACLS) within 6 months of hire.

Proactively serves as a patient advocate by utilizing courteous and professional etiquette at all times reflecting positive tone and speaking distinctly with poise, tact and assurance. Assures accurate and complete messages to appropriate personnel.
Assists in training, orienting, and supporting new colleagues. Work includes cross coverage in other clinic areas as team needs.
Document patient plan(s) of care, tests and examination results in the medical record as directed by the provider. Escalate non-routine issues, questions and/or concerns to the practice manager or healthcare provider(s).
Ensure safety checklists/quality controls are completed as required.
Other duties as needed and assigned by the manager.
Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.
MINIMUM QUALIFICATIONS
Must be comfortable operating in a collaborative, shared leadership environment.
Successful completion of a medical assistant program accredited by the Commission on Accreditation of Allied Health Programs (CAAHP) or Accrediting Bureau of Health Education Schools (ABHES).
Must obtain one of the following certifications listed for a Medical Assistant with their corresponding listed certification provider within one year of hire:
