Job Description Summary

This is a clinical role within the Primary Care department. Applicants must include an updated resume and cover letter detailing three years or more experience providing primary care, and three years or more experience providing gender affirming care.

This position is:

The FNP is responsible for providing exceptional primary care to gender-diverse patients across the lifespan.

NOTE: This position currently involves some remote work including conducting telehealth visits with patients. Remote work hours and duties are subject to change based on patient demand, federal and state changes to work regulations, and payor reimbursement guidelines.

Objectives & Responsibilities

Clinical Duties (80%):

  • Provide primary care to patients including management of chronic disease and episodic care. Collaborate with integrated behavioral health treatment interventions as needed.
  • All primary care providers are expected to provide gender-affirming care, including hormone provision, as part of our integrated care approach.
  • Other duties as assigned.

Administrative Duties (10%)

  • Complete medical record documentation, billing, prior authorization/ referral documentation and attend staff meetings.

Community Duties (10%)

  • This time is designed to allow clinicians to participate in educational and/or community-based initiatives of their choice.
  • These activities should serve to develop Transhealth staff, improve care for our patients/clients, or amplify trans and gender-diverse care, services, or knowledge within the community.

LICENSURE, QUALIFICATIONS & EXPERIENCE NOTES:

Licensure & Certification

  • Must be licensed as an Advanced Practice Registered Nurse (APRN) by the Massachusetts Board of Registration in Nursing.
  • Must hold an RN license in Massachusetts.
  • Must graduate from an accredited nurse practitioner program (Master?s or Doctorate in Nursing).
  • Must be board-certified as a Family Nurse Practitioner (FNP) by American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
  • Requires DEA registration to prescribe controlled substances.
  • Requires Massachusetts Controlled Substance Registration (MCSR).
  • Can practice independently in Massachusetts (does not require a physician agreement).

Experience & Qualifications

  • 3+ years prior experience in family medicine or primary care required.
  • 3+ years prior experience with gender-affirming hormone therapy is required.
  • Strong skills in patient-centered, affirming care required.
  • Completion of continuing education (CE) in transgender healthcare (e.g., WPATH, Fenway Institute training) is a plus.
  • Must be trained in inclusive, culturally competent care.
  • Strong understanding of hormone therapy protocols, lab monitoring, and patient-centered care for transgender and non-binary patients.

In this unique and life-changing career, couples reside in on-campus student homes and provide care, guidance, supervision, and support for a group of approximately 8–13 students living in the home.
