Hartford HealthCare’s Behavioral Health Network (HHC BHN), is seeking an exceptional Psychiatric Advanced Practice Registered Nurse with an interest in providing treatment for children and adolescents with mental illness and substance use disorders services at The Hospital of Central Connecticut (HOCC) ambulatory services located in Southington, CT
Some highlights of this opportunity include:
· Provide direct patient services to children and adolescents in an outpatient setting, including diagnostic admission evaluations, treatment plans, psychotherapy and pharmacotherapy, as appropriate.
· Completes timely documentation of assessments, clinical interventions and discharge summaries and updates records as specified in medical staff regulations.
· Monitor risk indicators and take action to address risk management issues in a timely manner to ensure client, staff and visitor safety, and to safeguard the Hospital from adverse events. Includes consultation with peers and acting as a resource/consultant to other members of the treatment team.
· Adheres to prescribing practices that are safe and evidence based. Obtains appropriate and informed consent for medications trials prior to prescribing.
· Acts as liaison and communicates effectively with outside providers, primary care physicians, schools and family members as needed.
· Provide supervision to selected non-physician health professionals.
· Participates in Medical Staff Committees and meetings as specified in Medical Staff regulations.

Introducing this excellent opportunity to join the obstetric team at Hartford Hospital. Our Labor and Delivery unit is a high risk referral center for the area and performs approximately 4,000 deliveries per year. Our Obstetric Triage unit evaluates more than 3,000 patients per year. As the volume and acuity of our patients has grown, we are looking to bolster our inpatient obstetric service with 24/7 APP coverage.
Strong clinical reasoning and excellent communication skills required. This role functions both independently and in collaboration with nurses, residents, and attending physicians. This position will report to the OB/GYN Hospitalist Director.
Advanced Practitioner Role Scope:
Care for patients admitted or being triaged across our Obstetric service line.
Help on Labor and Delivery with admissions, magnesium sulfate checks and labor checks when residents unavailable, during sign-out and periods of higher volume.
Rounding and evaluating our antepartum patients as needed based on volume and resident coverage. Evaluation may include concerns for labor, elevated BP’s, infection or medication orders. Help with biophysical profiles (BPP’s) for non-reactive NST’s and limited obstetric ultrasound
Typical postpartum care includes rounding, discharge planning, coordination of inpatient consults, responding to elevated blood pressures, postpartum or postoperative concerns, medication reconciliation and placement of Nexplanon.
If needed in triage you will evaluate pregnant patients for pre-term/term labor, rupture of membranes, bleeding, high blood pressure, pre-eclampsia, abdominal pain and a variety of other complaints. There is always an attending assigned to cover our service serving as backup to the residents and APP’s.

Advanced Practitioner Role Scope
The APP will be dividing their time between SNF setting and infusion/outpatient cardiology clinic.
The infusion clinic involves assessment, follow up, instruction and collaboration with the community cardiologist for the necessity of intravenous diuresis, thereby reducing the need for hospitalization, improving QoL, adhering and implementing GDMT.
The APP will not only be responsible for instituting a new infusion clinic needed across the central region outside of the current one established at HOCC, but also coverage of cardiac testing.
The APP will play a vital role in filling the gaps and needs of our heart failure population, educating our patients and families lifestyle modifications, GDMT and providing an additional resource to the affiliated cardiology practices.
Shift: Full Time – The hours split TBD by the needs of patient population.

Stroke Program Physician Assistant or Nurse Practitioner (APRN) Role Scope
· Provides holistic, quality and cost-effective advanced practice care for patients, families, and others in a target population, and participates in the medical management of patients, to achieve or maintain optimal health status, manage disease and reduce risk of illness.

About the Opportunity:
· As a vital member of the Cardiac Surgery team, the APP will deliver expert, compassionate care to patients in our 18-bed Cardiac Surgery intensive care unit (ICU), step-down unit, and floor settings. Responsibilities include:
· Managed preoperative and postoperative care for cardiac surgery patients.
· Perform bedside procedures including central venous and arterial line placement, intra-aortic balloon pump removal, etc.
· Provide care for patients undergoing advanced procedures, including heart transplants, ECMO (Extracorporeal Membrane Oxygenation), mechanical circulatory support, and transcatheter aortic valve replacement (TAVR).
· Conduct thorough patient assessments, developing individualized care plans, and closely monitoring progress.
