Training General Responsibilities:

• Enforce company policies and procedures
• Gather and evaluate feedback from customers, vendors, and employees to ensure training effectiveness

• Assist in the development, improvement, and standardization of company training programs
• Track, document, and report training outcomes and compliance to management
• Maintain current knowledge of safety programs and regulatory requirements

• Maintain the company training budget
• Tracking employee development from onboarding through their career to ensure continuous growth and proper training

• Any Company events as required
• Maintain accurate employee training records, certifications, renewals, and retakes
• Maintain company training resources such as the Employee Development Program, EDR Database, Company Website Training Center and shared training calendar
• Support company departments with safety, trade-specific training, and record-keeping submittals
• Plan and coordinate all training sessions and associated logistics (mockups, food and beverage etc.) for all company locations
• Assist in creating certificates and documentation of completed employee training

Office Staff Training:

• Maintain and track office new hire checklists for all company positions
• Ensure all required training items are being taught and completed on schedule
• Periodically communicate with and survey new hires on their training experience
• Provide managers with regular updates on new hire training progress and status
• Maintain updated training records

Field Staff Training:

• Coordinate, schedule, and maintain all company training programs for field employees
• Verify field staff are properly trained and qualified for assigned scopes of work
• Maintain training records for field staff and keep them up to date

Medical Assistants are an important part of our Care Teams. High levels of flexibility, energy, attention to detail, and problem-solving are required to be successful. Medical Assistants are expected to build relationships with Oak Street Health members. Medical Assistants work closely and collaboratively with their teammates to greet and room Oak Street Health patients. Medical Assistants are expected to assist in assessing a patient's health conditions, including performing routine diagnostic testing during appointments.

Core Responsibilities:

Ensure efficient patient flow by rooming patients in a timely manner and assisting the provider with staying timely on their schedule
Deliver an exceptional patient experience, and help patients prepare for their provider visit
Obtain vital signs, chief complaints and review medications
Deliver patient screenings per protocols
Order supplies and stock exam rooms
Conduct routine quality control checks, including infection control measures, equipment QAs, and check for expired medication and supplies
Ensure exam rooms are clean, safe, and ready for patient visits
Under direction of an RN, provider and/or protocol, provide telephonic support to patients
Under direction of an RN or Provider, offer MA appropriate “visits,” such as administering vaccinations and performing procedures, including but not limited to: ABI, spirometry, EKGs, etc.
Perform waived laboratory testing
Schedule diagnostic testing
Apply or assist with the application of durable medical equipment
Participate in care team meetings to discuss patient care and clinic operations
Other duties, as assigned

What are we looking for?
3+ years in a Medical Assistant role
CPR Certification required
Medical Assistant certification or registration required. Over 5 years of applicable, consecutive Medical Assistant experience may substitute for this requirement
EMR experience and proficiency required
2+ years of experience working with geriatric patients is preferred
Phlebotomy Technician Certification preferred
Strong communication skills and customer service orientation
Demonstrates problem-solving skills, accountability, and a flexible, positive attitude
Fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities we serve (where necessary and indicated in Job title)

Summary:

The RN Navigator in Population Health is responsible for coordinating and managing patient care across the healthcare continuum. This role focuses on improving health outcomes for populations by implementing evidence-based practices, promoting preventive care, and ensuring patients receive appropriate and timely interventions. The RN Navigator will work collaboratively with ACO and CIN Network providers, patients, and their families across CHRISTUS Health ministries to develop and implement individualized care plans. The RN Navigator will manage the length of service, promote efficient utilization of resources, and ensure that a well-organized and safe plan of care is established for every patient.

Responsibilities:

Summary:

The Specialty Clinical Pharmacist is responsible and accountable for the provision of safe, effective, and prompt medication therapy. Specialty Clinical Pharmacists provide proficient direct patient-centered care in a decentralized practice setting. In this clinical role, Specialty Clinical Pharmacists participate in all necessary aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care to the patients in their assigned areas. Pharmaceutical care services include but are not limited to assessing patient needs, incorporating age and disease-specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Specialty Clinical Pharmacists serve as departmental resources and liaisons to other departments, or external groups. They also conduct clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice. Specialty Clinical Pharmacists provide medication and practice-related education/training and actively serve as preceptors for Doctor of Pharmacy students and pharmacy residents. The Specialty Clinical Pharmacists will participate in the quality management program with the goal of improving services by monitoring processes, analyzing data, implementing interventions to improve, and evaluating the effectiveness of those interventions.

The Specialty Clinical Pharmacists will be responsible for the clinical management of patients on a variety of specialty therapies for medical conditions including but not limited to Hepatitis C, inflammatory conditions (gastroenterology, dermatology, asthma, and allergy), pulmonary conditions (idiopathic pulmonary fibrosis, cystic fibrosis), neurology (multiple sclerosis, migraine headache), dyslipidemia, osteoporosis, and oncology.

• Communicates effectively and appropriately with healthcare providers and caregivers (physicians, nurses, etc.)
• Actively participates in stewardship activities and restricted medication programs.

• Maintains proficiency in and actively engages in operational programs and specialty pharmacy areas as appropriate for job assignment.

Job Requirements:

Education/Skills

• Doctor of Pharmacy degree from an ACPE accredited school of Pharmacy is required.
• Must accrue American Society of Health-System Pharmacists (ASHP) Residency Preceptor status within 18 months of hire.

Experience

• PGY1 residency or 5 years of clinical practice (community, ambulatory care, PBM, or hospital) required.
• PGY2 residency preferred.
• Specialty pharmacy experience preferred.
• EPIC suite products experience preferred (EMR, Compass Rose, Willow Ambulatory)
• Significant knowledge and expertise in advanced pharmacy practice, specialty medication therapy management, and specialty evidence-based medicine in areas appropriate for clinical assignments required.

Licenses, Registrations, or Certifications

• Licensed pharmacist in the state of practice.
• Pharmacy preceptor license within 90 days of start date.
• Must obtain Certified Specialty Pharmacist (CSP) certification or other applicable board (e.G. BCOP, BCACP, BCPS, BCMTMS, BCGP) certification within 18 months of hire.
