- Ensures that residents with decubitus ulcers, vascular ulcers, and various other skin conditions receive appropriate treatment and/ or prophylactic treatment such as daily inspection, turning and activity, nourishment and incontinence care.
Qualifications
- 1-3 years of experience in a long-term care setting preferred, but not required.
- Current Registered Nurse (RN) licensure in the state.
- At least two years experience working in wound care preferred.
- CPR certification.

What Youβll Do
- Administer medications, as prescribed, and under the direction of a licensed nurse
- Promptly report to the nurse, a residentβs request/need for PRN medications.
- Promptly report to the nurse any adverse or concerning reactions to medications.
- Provide exceptional Personal service and daily care to residents.
- Communicate effectively with residents, staff management and families.

- Provides physical resident care to include all aspects of personal hygiene and grooming.

- Provides ongoing assessment and nursing care to patient in homes and health care facilities. - Follows current nursing professional standards of practice and agency approved protocols for home health.

Qualifications
Education

- Performs physical therapy treatments as prescribed by the physician, evaluating and treating patients by applying appropriate therapy techniques and modalities in client's home environment.
- Recommends modifications or changes in the patients' Plan of Care based on evaluation of their progress toward the patient goals.
- Educates/instructs/recommends patient, family members and other health team members of the proper techniques, for exercise program, precautions and self care activities to assure safety for caregivers and patient.
- Participates in the interdisciplinary team meeting (ITM) by exchanging team information on the progress and discharge plan in order to provide the highest level of continuity of care for the patient.
- Communicates with client, family members, clients' advocate(s), caregivers and physicians on patient's progress and discharge plan on a regular basis.
- Conducts home visits to implement the patient's Plan of Care in use of therapeutic and self care activities for the purpose of improving function and safety in order to allow the patient to return home at their highest level of function and independence.
- Participates in discharge planning by exchanging information with staff members, family members and physician to promote continuity of client care, in accordance with corporate policies, federal, state and local guidelines.

Qualifications
Education
- Current unencumbered license for the state of Ohio to practice as a Physical Therapist (PT) required.
Experience
- One year experience as a Physical Therapist in a hospital, long-term care facility, or home health agency preferred. New graduates welcome.
- Experience in the care of geriatric and physical therapy patients required.
- Knowledge of treatment modalities.

Job Description Summary:
This position leads the strategic vision, smooth operations, and the overall success of the balanced scorecard, including the planning, organizing, staffing, and establishing control systems for Radiology and Cancer Services. He/She develops and administers operating and capital budgets and performs other project work, as assigned. This position will work closely with hospital Medical Directors to ensure physician alignment and satisfaction within OhioHealth.Responsibilities And Duties:
25%Daily operations Responsible for the success of overall operations of the Radiology department, oncology service line, and the Cancer Center. Works directly with managers to ensure balanced scorecard metrics are met on a consistent basis. 24/7 responsibility for all radiology and oncology areas. Develops and administers operating and capital budgets, in partnership with appropriate management.
25% Strategic planning and execution Responsible for development of an overall strategic plan for the Radiology department, oncology service line, and the Cancer Center. Works directly with executive leadership, physicians, leadership team and associates to recognize opportunities for growth. Builds and maintains relationships with vendors and internal customers. Develops/maintains metrics to ensure success as anticipated. 3.
25% Management and staff development leads the management teams within Radiology and Oncology departments. Responsible for career development, coaching, and mentoring management and staff-level associates within Radiology and Oncology departments. Fosters and supports staff-level development within Radiology and Oncology. 4.
10%: System responsibilities Responsible for system initiatives as related to Radiology at the local level and the oncology service line. Β
10%: Physician Relationships Works collaboratively with all appropriate Medical Directors to promote positive relationships with Radiology and Oncology physicians. Responsive to physician concerns regarding operations and patient issues. Works directly with the physicians to collaborate on goals, metrics, new technology, etc.
Β 5%: Accreditation Responsible for maintaining current accreditations such as ACR, ACOS, NAPBC, JCAHO-DCS and others as deemed appropriate.
