GENERAL PATIENT CARE - Meets the comfort and hygiene needs of the patient.
TECHNICAL CARE - Assists with completion of patient testing and treatment procedures, sets up and primes IV tubing, 12-lead ECG, simple tracheostomy suctioning and care, changes dressings, performs cardiopulmonary resuscitation, obtains wound and throat cultures.
EQUIPMENT - Assists physician and professional staff in equipment supply and set up.
TRANSPORT - Assists with ambulating patients with assistive devices such as crutches and with transfer/transport procedures.
COMMUNICATION - Performs required documentation and communicates in a competent and effective manner to meet patient and/or family needs and regulatory requirements. Maintains strict confidentiality of patient, employee and department information to ensure patient rights.
QUALITY IMPROVEMENT - Utilizes time and resources effectively to promote quality and cost outcomes.
Position Requirements
Minimum Education:
Preferred: High School Diploma plus training acquired through work experience or education
Minimum Experience:

Licensure/Certifications:
Required: Certified Nurse Assistant, BLS (Basic Life Support) through AHA

Ensures that patient goals are set with patient/family, negotiates healthcare resources and educates patient/family regarding available options of care throughout the continuum. Demonstrates clinical competence to care for a specific patient population. Conducts daily multidisciplinary rounds demonstrating ability to identify and address care coordination barriers. Facilitate patient education for safe and effective discharge.
PROFESSIONAL DEVELOPMENT: Demonstrates leadership behaviors that enhance and support team functions. Mentors and coaches staff. Maintains competencies and participates in continuing education for self and development of colleagues and students. Participates in delivering professional programs/ courses/in-services, internal and/or external, and community events. Seeks opportunities to promote staff education, certification, clinical ladder, and award programs. Maintains national certification, as applicable, and membership in professional organization(s). Creates opportunities to advocate for the role of the professional nurse as a key member of the interdisciplinary team.
POSITION REQUIREMENTS:
Minimum Education: Required: Bachelor's Degree
Preferred Education: Master's Degree
Minimum Experience: Required: 3-5 years of previous job-related hospital experience.
Preferred Experience: 6-10 years of previous job-related hospital and Inpatient Care Management experience
Preferred Certification: ACMA, CMGT-BC, CCM
Recent Case Management experience is preferred.
DETAILS: Recent clinical experience in medicine, surgery, critical care, telemetry, or ED is required.
Compensation:
Salary Range: $35.89 - $62.91 per hour
Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
