The RN will manage a high-volume caseload, perform patient assessments, and ensure accurate documentation, including OASIS, in a fast-paced home health setting.
Key Responsibilities Provide direct patient care in home health settings .
Complete OASIS assessments and documentation accurately .
Manage a caseload of 5-6 patients per day .
Develop and implement individualized care plans .
Monitor patient progress and coordinate care with the healthcare team.
Educate patients and families on treatment plans and care.
Ensure compliance with home health regulations and documentation standards .
