Job Description:This position provides therapy services to adult patients, including evaluation and assessment, treatment, education, and consultation to improve patients’ abilities to their highest possible level. This position provides services as part of a care team assembled to assure optimal clinical and financial outcomes, under the direction of Essentia Health management and the referring physician. Education Qualifications:
- OT Education Requirements: Bachelor's Degree, Master's Degree, or Doctorate Degree in Occupational Therapy from accredited program
- PT Education Requirements: Bachelor's Degree, Master's Degree, or Doctorate Degrees from a physical therapy accredited program
PT/OT Key Responsibilities:
- Provide therapy evaluation, develop individualized treatment plans, implement evidence-based interventions, provide patient/caregiver education, and discharge planning for a full patient caseload.
- Provide all ancillary/administrative requirements for a full caseload of patients, including documentation, billing, scheduling management, and other administrative duties
- Demonstrate appropriate communication, professionalism and supervision of support staff (licensed assistants, aides, volunteers)

Outpatient Pelvic Floor:
Completion of Herman & Wallace Level I certification, or an alternative advanced training approved by Essentia Health leadership (may work with leadership on a training plan if you are not currently trained).
Schedule would be weekdays between 7:00am - 7:00pm. No weekend or holiday rotation is required for this role.
