GENERAL DUTIES:

The Patient Care Coordinator (PCC)/Supervisor of Nurses (SON diagnosis and treats human responses to actual or potential health problems through such services as data collection,

physical assessment, case-finding, health teaching, health counseling

and provision of care supportive to /or restorative of life and well-being as data collection, physical assessment, health teaching, health counseling, and provision of care supportive to or restorative of life and well-being and executing medical regimens prescribed by a licensed physician, dentist or other health care provider legally authorized under this title and in accordance with the commissioner’s regulation.

Under the guidance of the On-Site Director of Nursing (OS-DON), the Patient Care Coordinator/Supervisor of Nurses is responsible for directing and administering nursing care to patients. He/she coordinates activities through collaboration with physicians, administration, pharmacy, clerical, and correctional staff, etc.

He/she is guided by a thorough knowledge of principles of nursing based on biological, physical, and psychosocial sciences. He/she possesses a working knowledge of principles of leadership and utilizes that knowledge in guiding and directing nursing staff.

He/she assists the OS- DON in ensuring compliance with standards, policies and procedures of NYC Health and Hospital Corporation Correctional Health Services contracted with Prison Health Services, Inc. and the Department of Nursing. He/she will be working with adult male and females between sixteen to eighty and infants from birth to one year of age.

RESPONSIBILITIES:

  • Provides direct patient care as necessary. Using the problem-oriented progress notes, written to match each problem documented on the Problem List and organized using the SOAP format.
  • Coordinates the nursing care provided for patients within a given unit by:
  • Assigning appropriate nursing staff for patient care activities such as admissions, sick call, treatments, medications, punitive segregation rounds, follow-up, etc. Problems in staffing must be communicated to the On-Site Director of Nursing.
  • Utilize therapeutic communication skills and effective interviewing techniques to elicit pertinent historical information and to make appropriate referrals as required. Screen patients with mental health problems in an immediate manner and makes appropriate referral as required.
  • Making rounds to patient care areas for the purposes of assessing, planning, implementing, documenting, and evaluating unit nursing care delivery. Assist OS-DON in staffing needs. Provides guidance and assistance to nursing personnel in the delivery of patient care.
  • Assuring continuity of care on other shifts through written and/or verbal communication. Communicates special care needs of particular patients to the psychiatrist, On-Site Director of Nursing or the On-Island Administrator.
  • Collaborates with the OS-DON in the orientation of new staff on the unit. Evaluates the new nurse on an ongoing basis; writes a formal evaluation at the end of the probation period, and makes recommendations to the OS-DON.
  • Evaluates and provides feedback to staff on an ongoing basis and in an annual formal evaluation conference. Collaborates with the OS-DON

and/or Deputy Director of Nursing. Provides appropriate documentation and records of nursing personnel activities.

  • Identifies staff development needs and plans strategies to meet the needs in collaboration with available nursing resources.
  • Conducts meetings with his/her staff, in collaboration with the On-Site Director of Nurses, to share information, discuss problems and potential solutions, and to promote cooperative professional relationships among the staff.
  • Monitors the quality of patient care through quality assessment - improvements mechanisms.
  • Communicates, documents and collaborates on an ongoing basis, with the On-Site Director of Nursing concerning patient care and unit administration.

COORDINATOR / SUPERVISOR RESPONSIBILITES:

  • Coordinates the intra-disciplinary team providing comprehensive patient care.
  • Familiarizes self with and participates in defining, maintaining and Interpreting standards of nursing practice through the application of nursing policies and procedures according to the Prison Health Services, Inc. Policy and Procedure Manual and specific policies of the Correctional Health Services.
  • Identifies those components of the nursing plan, which may be delegated to allied nursing and other non-professional personnel, and provides teaching, guidance and supervision as necessary.
  • Assists with the unit orientation of nursing personnel through participation in
  • the precepting function.
  • Counsels nursing staff and directs information to the On-Site Director of Nursing. Provides documentation as needed on a timely basis.

Correctional Health Services

The New York City jail system is the second largest in the nation. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD).

Responsibilities:

Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatrist will be matched with the population that most closely meets their clinical expertise, desired work location and schedule and the clinical needs of the service. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical.

Responsibilities Include:

  • Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs.
  • Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information.
  • Evaluate patients for the need for a higher level of care, including hospitalization, and refer to a higher level of care as clinically appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care.
  • Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. Document all activities in the electronic medical record in a timely, concise and clear manner.
  • Integrate nursing and medical concepts in planning care for medically complicated psychiatric patients.
  • Provide consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment and/or the need for psychiatric medication.
  • Counsel patients using advanced psychotherapeutic techniques, including in group settings if needed.
  • Participate in team meetings with the treatment staff, on a daily basis if working on a mental observation unit and on a weekly basis if working in one of the clinics.
  • Engage in clinical supervision, as indicated, with the Supervising or Senior psychiatrist
  • Other duties as assigned by the Senior Psychiatrist or Mental Health Manager for the facility, Medical Director or Chief of Service for Psychiatry, or their respective designees.

Avery Hall is seeking a Development Manager to join our growing team. Reporting directly to the Vice President of Development, Director of Construction, and senior leadership, this individual will lead all facets of day-to-day development, design, and construction activities across multiple mixed-use and multifamily projects.

 

This role is highly collaborative and hands-on, engaging with design teams, consultants, contractors, and ownership throughout all phases of development. Key Responsibilities:

 

Development Management

  • Shepherd projects from initial concept through design development, 100% CDs, DOB filing, entitlements, procurement, construction, TCO/FCO, punchlist, and warranty.

  • Review, negotiate, and process all change orders, invoices, and cost proposals from the CM and consultants.

Financial & Reporting Responsibilities

  • Oversee the construction draw and requisition process, including verification of hard and soft cost expenditures.

Education

  • Work closely with Speech-Language Pathologist to ensure appropriate mechanically altered diets are provided for residents with dysphagia
  • Reviewing all menu changes to ensure they follow facility's policies/procedures & State/Federal guidelines
  • Ensure facility is in compliance with regulations and policies on weight monitoring
  • Communicate with interdisciplinary team on a daily basis to provide quality care to residents

Your responsibilities as a Behavior Technician at Lighthouse will include the following:

The Opportunity 

PPL is hiring an Operations Coordinator to help run best-in-class tournament operations and  player services across the season. This is a hands-on role on a small, nimble executive team.  You will coordinate many moving parts across vendors, officials, production partners, teams,  and players, while keeping standards high and details tight. 

Reporting line: Reports to the PPL COO. 


Core Outcomes (What success looks like) 

• Match days run smoothly, on time, and professionally.

Key Responsibilities 

1) On-Court Tournament Operations 

Coordinate match-day on-court operations across all events, including:

• Scheduling and coordinating head referee, chair umpires, glass cleaners, court  maintenance, and ball kids. 

• Ensuring courts and match environments meet PPL standards before, during, and after play. • Supporting the match-day run of show and resolving issues quickly and calmly. 

2) Live Scoring & VAR Coordination 

Own day-to-day coordination with PPL contractors/vendors for: 

• Live scoring operations (setup, staffing, process adherence, and troubleshooting). • VAR workflow and escalation paths, ensuring consistency across matches. • Communication loops so teams, officials, and production are aligned on decisions and  timing. 

3) Player & Staff Logistics 

In close coordination with production/event agencies: 

• Coordinate player and staff transportation and shuttle services. 

• Support hotel and accommodation logistics and related communications. • Maintain clear schedules, pickup windows, and contingency plans. 

4) Player Services & Experience Standards 

Coordinate with Player Services contractors to deliver a premium environment, including: 

• Locker rooms, training facilities, practice courts, physio/medical areas, lounges, and player  catering. 

• Practice court scheduling and standards. 

• Issue tracking and rapid resolution to keep player experience first class. 

5) Event Guides & Team Communications 

Develop and distribute player and team event overviews/guides for each event, including: 

• Key timelines, locations, transportation details, practice schedules, and on-site procedures. • Clear contacts and escalation paths. 

• Version control and timely updates as plans change. 

6) Awards, Trophies, and League Recognition 

• Coordinate production of event and season trophies. 

• Support development and operational execution of league awards (e.g., MVP), including  timelines, inputs, and delivery. 

7) Rankings & Competitive Data Operations 

Manage the weekly process for collecting required data and determining player rankings:

• Ensure inputs are complete, accurate, and on time. 

• Coordinate dissemination of rankings and related information across the league and teams. • Maintain clear documentation and consistency week to week. 

8) COO Administrative Support 

• Calendar and scheduling support. 

• Expense tracking and basic administrative follow-through. 

• Travel coordination support as needed.
