Position Summary
We are seeking a detail-oriented and experienced Patient Care Coordinator to join our team. This individual will serve as the first point of contact for new patient referrals and will be responsible for coordinating all aspects of the Intake process, including insurance verification, prior authorization, and communication with patients and providers. Key Responsibilities
- Coordinate the infusion referral process from start to finish, ensuring timely and accurate processing of all incoming referrals.
- Verify insurance benefits and obtain prior authorizations for infusion services, including both medical and pharmacy benefits.
- Serve as a liaison with referring providers, collecting necessary clinical documentation and keeping them informed of patient status.

Required Qualifications

General Summary of Position
The Physical Therapist provides physical therapy services to patients to include but not limited to screening and evaluation treatment planning treatment implementation treatment re-assessment and revision patient/client re-evaluation discharge planning and documentation in a variety of settings. The Physical Therapist participates as requested in program planning and protocol development and assists the team in meeting established goals.

Education
