Job Summary
B7E and 7E Step Down is a fast-paced, highly collaborative teaching unit serving medical patients suffering from an acute illness or injury, or experiencing an exacerbation of a chronic condition affecting one or more body systems. The unit has a strong respiratory focus but treats the full range of medical diagnoses. Step down serves critical care patients with additional interventions including, but not limited to, cardiac drips, insulin drips, Bipap, and chronic vents.
Our Nurses work in collaborative environments with strong interdisciplinary teams, focused on a patient centered care model in a richly diverse work space. Under the direction and general supervision of the Nurse Manager, the registered nurse utilizes evidence based practices in accordance with the State Nurse Practice Act, policies and procedures of the hospital and as directed by the medical staff. The registered nurse who through knowledge and ability uses the nursing process to develop a plan of care which includes standards of practice to meet the physical, emotional spiritual, cultural and educational needs of the patient and family.

Job Summary

POSITION SUMMARY:
The Cardiac Rehab RN supervises medically prescribed exercise therapy appropriate to the patient’s age, medical and emotional/mental status. The RN provides education classes, concentrating on client’s understanding of the cardiovascular system and heart-healthy lifestyle modifications necessary to maintain optimum health. The RN is responsible for orientation of cardiac and pulmonary patients and participating in Quality Improvement and KPIs. Assesses patient and/or family learning needs, including the ability and readiness to learn. Identifies and arranges appropriate resources to assist in the educational process.
Participates in department QI and KPIs, data collection and reporting and Policy and Procedure maintenance.

Conducts education classes for the patient and significant others as per education calendar according to appropriate teaching modules and documents in the patient’s chart.

Job Summary
Heart Failure Transitional Care Nurse Coordinators participates in an expanded nurse role as an integral member of the Heart Failure Team. They are at the forefront of the Heart Failure Disease Management Program and work in a collaborative environment with strong patient-centered interdisciplinary teams, focused on a patient-centered care model in a richly diverse work space. The nurse coordinator must possess our institutional values of integrity, kindness, safety, nurse excellence, leadership, professionalism, and loyalty while providing high quality safe care. Under the direction and general supervision of the Heart Failure Program Manager, the registered nurse coordinator uses high-level critical thinking skills and expertise in the safe administration of various heart failure intravenous medications aimed at decongestion while also balancing and providing the framework of a robust comprehensive advanced heart failure management program. The Heart Failure Transitional Care Nurse Coordinator utilizes evidence based practices in accordance with the State Nurse Practice Act, policies and procedures of the hospital and as directed by the medical staff. With a goal of reducing readmissions and improving quality of life for patients, coordinators use expert knowledge and the nursing process to develop a plan of care which includes standards of practice to meet the physical, emotional, spiritual, cultural, and educational needs of the patient and family. The coordinator provides specialty and individualized care, implementing interventions including heart failure education, medication education, dietary education and on-going counseling, promotion of self-care, and adherence to a therapy plan.
Key Areas of Responsibility
• Independently obtains and performs assessments while safely triaging and guiding a patient’s clinical course. Presents patient history and clinical presentation to a Provider. Establishes, implements, and evaluates an individualized plan of care with timely and appropriate clinical advice and interventions.
• Perform extensive review of medications and eliminates inadvertent inconsistencies across transitions in care. Communicates effectively with Providers to validate and verify medications and ensure medication safety and improved effective medication reconciliation and follow-up care.
• Facilitates coordination of care across the continuum and collaborates with Advanced Practice Providers and Physicians to utilize therapy protocols. Performs IV and medication therapy, assessment, and evaluation of the patient
• Participates in the overall program growth, referral base, outreach, protocol establishment and re-evaluation
• Participates in research and present programmatic milestones and accomplishments in various venues
• Leads heart failure support groups, educational sessions, and develops Heart Failure newsletters, and other activities/ materials as needed.
• Coordinates the Heart Failure patient experience throughout the Hartford Healthcare continuum based on the individual needs of the patient.

Bachelor of Science Nursing Degree required
Experience
Minimum of 3 years’ acute care experience preferred.

Responsibilities:
• Completes Initial Assessment for patients referred to the outpatient cardiac
rehabilitation program
• Identifies indications for participation, contraindications
for participation and specific monitoring needs and risk factor modification
goals for each patient.
• Completes patient education/counseling and documents treatment plan, including measureable goals, tailored to the individualized goals/needs of each patient referred to the program.
• Evaluates, monitors and documents multiple patients response to exercise therapy prior to, during and after exercise therapy. This ongoing assessment includes blood pressure, heart rate, rate of perceived exertion, telemetry/EKG, and may also include oxygen saturation, daily weight, blood sugar and ventricular assist devices
• Reports any significant participant acute conditions (e.g: dysrhythmias, blood pressure, angina symptoms, weight gain) to the supervising physician immediately. Reports ongoing, abnormal trends to physician/provider as needed (ex: elevated blood pressure; abnormally low blood pressure, episodes of hypo/hyperglycemia) via event report.
• Initiates ACLS measures and/or emergency care according to hospital policy.
• Completes tasks related to patient recruitment, insurance pre-certification, registration, and records management (Scott Care, Epic) as needed.
• Keeps current in the field of cardiac rehabilitation through journals, in-services, mock codes, conferences and staff meetings

Care Management is a collaborative practice model including patients, nurses, social workers, physicians, healthcare team members,, caregivers and the community. Incumbents of this position are professional Registered Nurses that utilize the nursing process (assessment, planning, intervention, documentation, and evaluation) to determine and facilitate the most appropriate level of care and identification of discharge needs. Collaborates with physician to ascertain medical treatment plan and with nursing, other health care team members and health plans to fulfill the treatment plan in the highest quality, cost effective manner. Responsible for ensuring customer satisfaction (patient, family, physician, others), maintaining patient confidentiality and being sensitive to the age and cultural specific patient needs for comfort, privacy and generalized care. .

- Responsible for complete and thorough patient and family assessments for initial and on-going care and discharge planning, addressing physical, psychosocial, religious, cultural and educational aspects. Identifies and communicates appropriate information gathered from patient, family, chart, community agencies and colleagues to other members of the multidisciplinary health care team. Actively coordinates patient care including the sequencing and scheduling of tests, procedures, and consultations. Works closely with physicians to coordinate hospital services from pre-hospital through post discharge recovery.
- Ensures patient and family understand the diagnosis and planned course of treatment and determines and communicates discharge needs. Identifies gaps / barriers to care and facilitates interdepartmental communications to expedite appropriate changes. Additionally, incumbents are responsible for assessing any financial needs of the patient / family, including assisting in the clarification of benefit plans.
- Assists Social Worker Care Managers with highly clinical complex patient needs.
- Responds expeditiously to emergency needs of patient and families in crisis by providing appropriate interventions necessary to support and stabilize.
- In conjunction with patient, family, payers and other members of the health care team: formulates and implements a discharge plan to address assessed needs and patient/family concerns,; evaluates the effectiveness of the plan in meeting the established care goals; and revises the plan as needed to achieve desired outcomes.
- Refers patients to a variety of resources including, but not limited to, Visiting Nurse Agency, Skilled Nursing facility, Long Term Care Hospital, Rehab Facility, Hospice Care, Durable Medical and Respiratory Care providers.
- Assures Follow-up care with Providers has been arranged with confirmation of the patient’s ability to access the scheduled appointment.
- Telephone Contact to patient’s who are at a high risk for readmission within 48 hours of discharge to verify the follow-up services has occurred as planned; Patient and Family understand and are following their discharge instructions and Patient and family are able to follow-up with their provider for evaluation.
- Pro-actively advocates for patient care issues to ensure that overall quality and type of care is sensitive to each specific patient/family’s needs.
- Analyzes patient care trends and actively seeks out and collaborates with the care team to improve overall quality and efficiency of care
- Seeks out and/or provides peer consultation about cases that are presenting problems and or experiencing significant deviations from the plan of care. Attends and participates in extended stay rounds, daily outcome planning rounds, Case Management Team meetings and peer reviews regarding management of their caseload.
- Participates in quality improvement and evaluation processes related to the Care Management function. Abstracts data from the medical record to enable the review of selected hospital core measures or for review by external agencies such as the JCAHO, CMS and/or QIO. Collects and trends a variety of data to identify avoidable hospital days and/or process improvement opportunities.
- Meet required job specific competencies for assigned unit, including mandatory educational requirements. Maintains professional growth and development through seminars, workshops and professional affiliations to stay current on the latest trends related to position. Participates in staff meetings, educational programs and in-service meetings to maintain competencies. Complies with hospital policies and procedures related to safety, infection control, attendance, sexual harassment, dress code, corporate compliance, confidentiality and others. Communicates to Administrative Director and/or Management staff needs identified for self, the unit and other team members.

Qualifications
EDUCATION/CERTIFICATION:
Must be a graduate of an accredited school of Nursing and have a current Registered Nurse license with the state of Connecticut.
Must be trained in de-escalation techniques, and receive certification within the orientation period of hire.
Must receive RQI certification within three (3) months of hire and maintain clinical competency during employment within this role.
COMPETENCIES:
Must be able to communicate effectively using the English language in both verbal and writing situations.
Must possess basic computer skills.
Must be capable of taking swift and efficient action, including patient restraint when responding to patient emergency codes.

Job Description
Under provider direction, the RN provides skilled nursing care to patients in a variety of clinical settings. Scope of responsibility is characterized by use of nursing process to assess, plan, intervene and evaluate human responses to actual or potential health problems utilizing appropriate practices, standards, protocols and guidelines. This position reports to a Practice Manager.
· Provides office-based nursing care in collaboration with provider
· Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care through appropriate nursing evaluation and intervention

· Triages telephone calls and displays ability to initiate appropriate nursing intervention in prompt, precise, and professional manner

The Hartford HealthCare Cancer Institute focuses on offering our cancer patients an unparalleled network of coordinated services– all under one roof. Registered Nurse (RN) will work in our outpatient Infusion Centers providing holistic, quality nursing care for patients/families and/or others for a patient population within the Infusion Centers. Responsibilities include but, are not limited to the following:
• Demonstrates excellent ability to perform patient assessments, and provide symptom management as well as patient and family education.
• Practices safe administration of chemotherapy/biotherapy, infusions, injections, and transfusions.
• Acts as part of a multidisciplinary team to provide continuity of care throughout various stages of treatment and follow-up including, but not exclusive to: providers, social work, dietician, integrative medicine, pharmacy, navigators, laboratory, radiation therapy, genetics, and clinical research.
• Collaborate with provider offices to schedule patients for infusion visits.
• Triage of patients through use of pathways and collaboration with physicians for toxicities, symptom management and needs for referrals within the RN scope of practice.
• Manage a caseload of patients receiving a variety of infusion and oral therapies on an outpatient basis.
• Demonstrates high level skills to manage complex workload effectively to provide safest, high quality care.
•Demonstrates an excellent ability to communicate through various methods and with various audiences to achieve desired outcome(s).
•Participates and engages in continuous professional development, safety behaviors and focus on the patient experience.
• Participates in the implementation of clinical support programs to ensure optimal care throughout the health-care continuum
Qualifications
Minimum Requirements:
• Current Connecticut RN License
• Current OCN Chemotherapy/Biotherapy card (or within first month of hire)

•BSN strongly preferred
Experience:
•Two (2) years of Previous Oncology Nursing experience strongly preferred
