Role Qualifications:

  • Graduate of an accredited program in Radiologic Technology
  • Ability to work with patients of all ages.

Radiology Duties:

  • Prepare patient for x-ray, CT scan, explain procedure to patient and/or family, prepare room, equipment, and materials needed.
  • Position patient and arrange immobilization and support devices for patient
  • Practice radiation protection techniques to limit exposure to patient and medical staff.
  • Adjust equipment controls, determine proper voltage, and exposure time.
  • Administer routine x-ray exams.
  • Observe patient and machine during procedure. Report unusual occurrences and record patient’s condition.
  • Document patient chart with procedures performed.
  • Clean and disinfect radiology room and equipment, maintain and order supplies for radiology.
  • Perform radiology quality assurance tests and maintain records.
  • Assist with front-end registration staff or additional clerical duties as necessary.

Laboratory Duties:

  • Ability to learn and master immunoassay instrument procedures including running calibrations, controls, reagents, detergents, sample testing and all consumables.

  • Complete training in use of laboratory equipment and required documentation to comply with all OSHA and CLIA standards.
  • Patient interview and documentation relevant to completion of lab required paperwork.
  • Decanting of urine specimens into sample tubes and completion of immunoassay testing per protocols within standard operating procedures (SOPs)and/or instruction manual for laboratory machines.
  • Perform laboratory testing on patient samples per SOPs and instruction manuals, including proper daily controls and intermittent calibrations as required.
  • Perform corrective actions as required and simple routine maintenance of lab equipment as instructed in training, SOPs, and equipment instruction manuals.
  • Prepare report of machine assays in such a fashion that can be placed into the physician’s medical records.
  • Contact equipment servicer, Lab Director, Clinical Consultant, or technical consultant as needed for assistance.
  • Dispose of hazardous and non-hazardous waste per OSHA guidelines
  • Properly prepare labs billing sheets, and supply billing company with copy of patient insurance face sheet and insurance ID card by FAX transmittal or as otherwise required.
  • Maintain proper inventory of consumables, office supplies, and paperwork necessary to continue daily operations.
  • Assist in Labs’ operations as directed by Nurse Manager or other delegated authority. Such duties may include but are not limited to inventory management, accounting and marketing.
  • Report immediately to Nurse Manager any clinical, equipment, or business issues that arise during operations that are unusual, hazardous, disruptive, or that require corrective actions.

Summary:

The RN Navigator in Population Health is responsible for coordinating and managing patient care across the healthcare continuum. This role focuses on improving health outcomes for populations by implementing evidence-based practices, promoting preventive care, and ensuring patients receive appropriate and timely interventions. The RN Navigator will work collaboratively with ACO and CIN Network providers, patients, and their families across CHRISTUS Health ministries to develop and implement individualized care plans. The RN Navigator will manage the length of service, promote efficient utilization of resources, and ensure that a well-organized and safe plan of care is established for every patient.

Responsibilities:

  • Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.
  • Care Coordination of Complex/Chronic conditions: Manages and coordinates care for patients with chronic conditions, complex medical needs, and assists with Discharge Planning: Helps plan and coordinate the discharge process for members leaving hospitals or long-term care facilities, ensuring a smooth transition to home or another care setting.
  • Care Coordination - Transitions of Care: Outreach to patients that qualify for Transitions of Care (IP Discharge) and ensure they understand their medications, educate patients on managing their conditions and knowing when to seek help, stressing the importance of scheduling and attending follow up appointments, and teaching them to recognize the signs that their condition might be worsening.
  • Patient Assessment: Conduct comprehensive assessments to identify patient needs, barriers to care, and social determinants of health.
  • Care Planning: Develop and implement individualized care plans based on patient assessments, clinical guidelines, and patient preferences. Focuses on reducing preventable admissions, readmissions, and preventable ED visits by supporting discharge planning to the next level of care and educating patients about the appropriate setting for care.
  • Advocacy: Serve as an advocate for patients or clients, helping them to navigate the healthcare system, understand their treatment options, and access the services they require.
  • Collaboration: Work closely with healthcare providers, social workers, and community resources to ensure a holistic approach to patient care.
  • Monitoring and Evaluation: Track and communicate to PCPs and specialty care providers any significant changes to members' concerns, along with any updates on members’ status.
  • Documentation: Maintain accurate and timely documentation of patient interactions, care plans, and outcomes in the electronic health record (EHR) system.
  • Quality Improvement: Participate in quality improvement initiatives to enhance patient care and population health outcomes.
  • Compliance: Ensure compliance with all regulatory requirements, organizational policies, and best practices in case management. Promotes a positive work environment by displaying a caring, sensitive approach to others, as evidenced by listening, understanding, and responding to the needs of patients, colleagues, and supervisors.
  • Must have strong clinical assessment skills.
  • Must have excellent communication and interpersonal skills.
  • Must be able to work independently and as part of a team.
  • Must be proficient in keyboarding and EHR systems.
  • Performs other duties as assigned.

Job Requirements:

Education/Skills

  • Bachelor’s Degree in Nursing preferred

Experience

  • 3 years of clinical experience required
  • 2 years of case management experience required
  • Experience working in a primary care value-based care organization is required
  • Knowledge of population health management principles is required

Licenses, Registrations, or Certifications

  • RN license in the state of employment or compact is required
  • One of the following certifications is required within 2 years of hire
    • Certified Case Manager (CCM) by CCMC
    • Nursing Case Management Certification (CMGT-BC) by ANCC

The primary purpose of a Multi-Modality MRI Technologist is to perform all the imaging and clinical duties of a MRI, CT, and Radiologic Technologist, in addition, performs special imaging techniques and assists the Radiologist with interventional procedures that require imaging guidance.

• Graduate of an approved Radiologic Technologist Program and meets the MQSA guidelines
• Current Basic Life Support certification
• Current American Registry of Radiologic Technologists« certification — ARRT (CT, MR, R) required
• Texas Medical Board certification
• Work Experience: 1 year preferred
• ARRT (MR); employee will be required to obtain ARRT (CT, or R) license within 1 year of completing orientation.

Job Title: Dry Loading Supervisor

Pay: $47,500 (+ bonus)

Location: Irving, TX (onsite)

Note: This operation runs 24/7, and individual schedules are determined by volume and client needs. Weekend availability is required and the facility is only closed three days each year.

Job Purpose

The Loading Supervisor is responsible for providing supervisory support to the loading and shipping operations within the warehouse. This role ensures efficient coordination of loading activities, adherence to operational standards, and collaboration with internal teams to meet shipping schedules and productivity goals.

Essential Duties and Responsibilities

  • Supervise associates within the loading department, including shipping personnel and warehouse office clerks.
  • Conduct daily roll calls and record employee schedules and time entries.
  • Monitor and report attendance issues.
  • Coordinate with dispatch operations and yard personnel to manage trailer movement and loading activities.
  • Manage dock door assignments and trailer placement within the yard.
  • Assign duties to employees and schedule breaks, work hours, and vacation time.
  • Conduct periodic employee evaluations in accordance with HR policies and procedures.
  • Support warehouse floor supervisors and associates with warehouse policies, departmental procedures, and job duties as needed.
  • Investigate and follow up on customer concerns or complaints, review discrepancy reports, and resolve issues to maintain strong customer relationships.
  • Ensure store departure schedules and loading deadlines are met.
  • Delegate loading and down-stacking assignments to warehouse staff.
  • Oversee sanitation practices throughout the warehouse and office areas.
  • Ensure proper load sealing procedures and compliance with security policies.

  • Work closely with staff to maintain and communicate quality and productivity standards.
  • Maintain flexibility to work various shifts based on operational needs.

Required Skills and Abilities

  • Excellent communication and leadership skills with the ability to motivate and supervise a diverse workforce.
  • Self-starter with the ability to manage and oversee warehouse data entry processes as directed by management.
  • Strong computer proficiency, including warehouse management systems (WMS).
  • Ability to work collaboratively across departments and effectively manage operational priorities.
  • Prior warehouse supervision experience preferred, particularly within wholesale or distribution environments.

Education and Experience

  • Some college-level management coursework preferred, or a minimum of 2 years of warehouse supervisory experience, preferably within a wholesale or distribution environment.
  • Strong computer skills, including proficiency with Microsoft Office (Excel, Word) and warehouse management systems.
  • Excellent written and verbal communication skills with the ability to interact effectively with all levels of management and staff.
