Registered Nurse Care Manager
SUMMARY: The RN Care Manager is responsible for assessing the care needs of participants, provides nursing and healthcare interventions, and evaluates outcomes of care of participants on an ongoing basis. In collaboration with the interdisciplinary team (IDT), develops plans of care to meet participants' needs. Delegates tasks to clinic, center, and homecare aides according to participant needs and care plans. Collaborates and communicates with the primary care provider, clinic staff, and other members of the IDT. Provides care to participants in the clinic, center, and participant homes as needed.
ESSESNTIAL DUTIES AND RESPONSIBILITIES:
Provide high quality clinical care and serves as a member of the PACE interdisciplinary team (IDT).
Provide nursing care in the center, clinic, contracted facilities, and participants' homes according to each participant's plan of care. (NJ: in accordance with the State of New Jersey Nursing Practice Act, N.J.S.A. 45:11-23 et seq., as interpreted by the New Jersey State Board of Nursing, and written job descriptions. Services provided shall be documented in the participant's medical record ).
Participate in 24/7 on-call process for triage of participants and their needs.
Assess, plan, and coordinate participants' home care services. Provide input to the IDT in developing home care plan interventions. The nursing care needs of the participant shall be assessed only by a registered professional nurse.
Monitor participants' acute and chronic care needs in all settings. Provide coordination and direct care as indicated to promote continued care in the community or promote optimal institutional care (Assisted Living, Nursing Home, Hospital, etc.) as needed.
Ensure timely follow-up by providers on specialist visits and will assist with obtaining specialist reports, facility documentation, and labs if needed.
Reconcile facility MARs for your assigned panel of participants monthly to ensure accuracy and medication adherence, notify provider of any discrepancies.
Notify participants of normal test results.
Complete timely and accurate nursing assessments in accordance with policies and regulatory requirements.
Implement nursing-related care plan interventions.
Teach participants, caregivers and families about self-care, medications, healthy lifestyles, infection control and safety to promote optimal health and safety.
Review and revises goals and approaches to participants' care in coordination with participant , family, caregiver and interdisciplinary team.
Works collaboratively with the interdisciplinary team (IDT) to develop and implement comprehensive plans of care for participants.
Develop and maintain positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.
Participate in all interdisciplinary team meetings.
Assist the interdisciplinary team members in understanding the significant nursing, self-care and functional needs related to the participant's health problems.
Supports OT as a back up to performing the duties of Home Care Coordinator on the IDT as needed/when assigned. May perform the duties of other IDT members based on professional licensing, competencies, and experience as needed
Actively participates in utilization review meetings and quality improvement projects / meetings.
Evaluates the competence of CNAs and Home Care Aides and delegates tasks and duties to them as indicated.
Participates in family meetings, staff meetings, in-service and training and orientation programs as required.
Follows all PACE Program Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
Protects privacy and maintains confidentiality of all company procedures and information about employees, participants and families.
Practices standard precautions and follows PACE Program Infection Control protocols.

EXPERIENCE, EDUCATION AND CERTIFICATIONS:
Bachelor of Science in Nursing Degree preferred.
State RN License required
NJ: Licensed by the New Jersey State Board of Nursing.
BLS required (have within90 days of employment).
1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role).
Experience in home care, long-term care and /or managed care preferred.

Skilled Maintenance Technician
We’re looking for a skilled, hands-on Maintenance Technician who takes pride in keeping machines running and facilities in top shape. This role owns the maintenance, repair, and preventative care of our fill and pack equipment, as well as overall building maintenance across two locations. If you love troubleshooting, getting greasy, and being the go-to “fix it” person, this role is for you.
Key Responsibilities
Equipment & Production Support
- Maintain, troubleshoot, and repair fill and pack machinery to minimize downtime
- Perform preventative maintenance (PMs) on all production equipment per schedule
- Diagnose mechanical, electrical, pneumatic, and hydraulic issues
- Support production during changeovers, startups, and unexpected breakdowns

Essential Dutiesand Responsibilities (as an Independent Contractor)

Responsibilities Include:
- Plan and deploy Enclave Boundary Defense systems and programs including firewall, proxy server, cloud services and other devices and applications.
- Plan and deploy De-Militarized Zones (DMZs) for each managed firewall as required.
- Provide onsite and remote support to boundary security and programs, which include Checkpoint Firewalls, Aviatrix Product Suite, various types of cloud security, security controls and any other platforms.
- Participate in the development, testing, and implementation of firewalls and proxy servers
- Verify devices are configured in accordance with DISA Security Technical Implementation Guides (STIGs).

KEY RESPONSIBILITIES
Property and Asset Management
- Work cross-functionally across all teams to ensure eviction processes are performed according to company policy.
- Oversee all aspects of the apartment communities to maintain financial goals and metrics, ensure the properties are well-maintained and meet company standards.
- Regularly inspect your designated assets’ physical condition, ensuring great curb appeal, clean common areas, and are a safe environment.
- Coordinate and supervise repairs, and upgrades to maintain property enhancements and product availability.
- Monitor and manage budgets ensuring cost-effectiveness without compromising quality.

- Resident Services (Renewals & Billbacks): Collaborate with Resident Services to ensure timely renewal pricing is available and communicated, and that move-out damages are documented and submitted promptly to support accurate billbacks.
- Accounts Receivable (AR): Work alongside the AR team by providing on-the-ground resident context, assisting with delivery of hard notices when needed, and supporting collection efforts through direct resident communication.
Compliance and Reporting
- Conduct all business in accordance with company policies and procedures, Fair Housing, Americans with Disabilities Act, Fair Credit Reporting Act, and all other laws pertaining to multifamily communities to ensure compliance with local regulations, company policies, and industry standards.
- Maintain accurate records for resident interactions, leasing, maintenance, and other operational activities.
- Prepare and present regular reports on property performance, resident satisfaction, and operational efficiency to senior management.
EDUCATION, EXPERIENCE, AND SKILLS

BASIC FUNCTION:
CASTO's Payroll Coordinator ensures accurate and timely payroll processing, maintains organized records, and supports essential HR functions. This role is an integral part of the human resources team.
MAJOR DUTIES AND RESPONSIBILITIES:
Payroll:
- Manage the timekeeping system and assist supervisors with processing time
- Enter all changes in payroll system, ensuring integrity of the data
- Transmit bi-weekly payrolls
- Import files and run scheduled and ad hoc payroll reports
- Cut manual checks as needed outside the normal payroll cycle
- Maintain accurate records in the company's electronic filing system
Personnel Changes:
- Track the receipt of associate documents and manage new hire checklists
- Process terminations, including drafting termination letters and processing termination checklists
FMLA and Medical Leave:
- Process and track all medical leave and ensure the appropriate documents are sent and received
- Work with our third-party FMLA administrator to manage FMLA claims
Other duties and responsibilities:
- Coordinate annual compensation review by preparing market data and salary sheets
- Protect the integrity of HR's electronic filing system by ensuring documents are current and entered without error
- Prepare information to submit in response to unemployment claims
- Respond to employment verification requests
