  • Take and record residents' vital signs (blood pressure, temperature, pulse, respiration) and monitor for changes in condition.
  • Provide assistance with activities of daily living (ADLs) when needed, such as bathing, dressing, feeding, mobility, hygiene.
  • Perform basic wound care, dressing changes, catheter or ostomy care (if within scope and facility policy).
  • Collect specimens for lab tests (e.g. urine, sputum) as ordered.
  • Observe and document residents' physical and emotional status; report any significant changes to supervising RN or physician.
  • Maintain accurate and timely documentation of care provided, treatments administered, and resident observations in medical records.
  • Communicate with residents, families/POAs, and the healthcare team about resident status, care needs, and any concerns.


QUALIFICATIONS:
  • Valid, unrestricted LPN license in Connecticut.
  • Graduate of an accredited practical/vocational nursing program.
  • Current CPR/BLS (or equivalent) certification as required by facility policy.
  • Strong interpersonal and communication skills; ability to work well with residents, families, and the care team.

Job Summary

Clinical Supervisor of Behavioral Health oversees a team of 5 full time master's level clinicians, a case manager, and a recovery support specialist who provide treatment within an acute inpatient psychiatric unit.  Partnering in multidisciplinary leadership with a nurse manager and medical director to provide programmatic oversight of the unit and treatment for patients 18 years of age and older. Additional responsibilities include providing clinical supervision to the clinical team, supporting the clinical development of clinical trainees placed within the inpatient units, and engaging in quality improvement projects at the direction of the clinical director in accordance with best practices and regulatory requirements.  Clinical services include individual, group and family therapy. 

This is a key supporting member of the management team that oversees day-to-day program operations and is responsible for the supervision of assigned clinical care activities to monitor standard work and meet regulatory requirements.  Career growth opportunities. 

Key areas of responsibility: 

Unit and Personnel Management

  • Supervise all clinical care, including use of behavioral health interventions, emergency management and monitors compliance with requirements for treatment planning and transition planning.
  • Hires, trains, provides coaching and gives corrective action, delegates tasks to ensure productivity and educates staff.
  • Assesses staff performance and develops clinical team.
  • Identifies opportunities for operational improvement and plans to implement them in collaboration with Program Manager.
  • Provides clear goals and expectations for staff and holds them accountable. Sets and measures results with respect to established objectives.

Direct Patient Care

  • When required, conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate.
  • Provides direct patient care that may include: case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end-of-life care, referral and discharge planning.

Clinical Excellence

  • Leads administrative and development activities including but not limited to: team meetings, clinical supervision of staff and staff education.
  • Provide social work field supervision to second year social work students


Qualifications

  • Master's degree in social work, counseling or marriage and family therapy required.

  • Current Connecticut license; LCSW, LPC, LMFT required.

  • 1 year of management experience in a healthcare related setting.

     

  • 2 years of experience as a licensed Clinician; LCSW, LPC, LMFT.

  • Previous experience in clinical setting with Adults or Child/Adolescents

  • Computer proficient

  • Ability to assess problems, determine solutions and make decisions within a timely manner

  • Utilize supervision and team approaches in the context of patient assessment and clinical interventions

  • Excellent communication skills, with the ability to establish relationships within and outside their entity

  • Focused on personal, patient/client and unit/staff safety

Job Summary

Behavioral Health Clinician works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services in the Inpatient department. Clinical services include individual, group and family therapy. 

Key responsibilities of Behavioral Health Clinician: 

  • Conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate.

Qualifications

If you meet these qualifications, we would love to meet you:  

  • Associate degree preferred
  • Certified Hospice Nurse (CHPN) preferred
  • Three+ years' experience in hospice, home health, general medical surgery, oncology, or patient population-specific nursing (i.e., geriatrics, pediatric, etc.)
  • Minimum of one year of supervisory experience
  • Possess and maintain valid CPR certification while employed in a clinical role (preferred)  
  • Must be a licensed driver who can travel to all business locations  
  • Meet the regulations and requirements of the state(s) in which the program provides services  

Responsibilities:   

As a Hospice Registered Nurse Team Director, you will: 

  • Supervise direct patient and family care team members, in collaboration with program site leadership
  • Responsible for scheduling , payroll, and other administrative duties
  • Ensure all initial assessments and required reassessments are conducted by qualified team members within required time frames, including documentation submissions, and/or synchonization
  • Oversee and support the Interdisciplinary Group (IDG) in development, implementation, and review of the interdicisiplinary plan of care for each patient and family
  • Collaborate with program site leadership memebrs to monitor, manage, and mentor staff in patient care activities, productivity, and documentation per related policies and protocols
  • Pariticpates and promotes Back to Bedside efforts in reinforcing Case Management skills, and knowledge and attitude per protocol for all disciplines
  • Collaborate with site leadership to support staff education efforts
  • Facilitates IDG meetings and provides ongoing IDG Support such as triaging incoming calls, provides caller support and assistance, and performs quality calls
  • Participates in or lead quality improvement activities, including data collection and aggregation as able
  • Reinforces and supports all internal and external communications
