What you will do:

Located in the heart of Clifton Park, Schuyler Ridge is a 120‑bed skilled nursing facility offering:

Sub‑Acute Rehabilitation

A small, focused rehab unit designed for short‑term recovery. Nurses enjoy a more manageable patient load, stronger patient connections, and less burnout.

Long‑Term Care (LTC)

LTC is divided into smaller, neighborhood‑style units that create calm, supportive environments. This structure helps nurses build meaningful resident relationships and deliver consistent, high‑quality care with strong team support.

Memory Care

A dedicated, smaller‑scale Memory Care Unit tailored for residents with dementia. The environment is structured, safe, and staffed by teams trained in dementia best practices—allowing nurses to provide compassionate, individualized care without the stress typical of large, high‑acuity memory programs.

Key Responsibilities

  • Provide direct, compassionate nursing care under RN supervision
  • Monitor vital signs, administer medications, and assist with treatments
  • Support residents with daily living activities and overall well‑being
  • Collaborate closely with nursing leadership and interdisciplinary team members
  • Communicate effectively with residents, families, and providers
  • Document care accurately in electronic medical records
  • Maintain safety, cleanliness, and infection control standards
  • Serve as a professional, caring representative of the nursing team

Required Qualifications

  • Current LPN licensure in New York State
  • Ability to remain calm, organized, and effective under pressure
  • Prior long‑term care experience preferred
  • Strong communication and interpersonal skills
  • Effective team collaboration abilities
  • Solid time‑management and prioritization skills
  • A genuine passion for providing respectful, compassionate resident care

Pay Range: $24.00 - $31.10 per hour

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
