Licensure / Certification / Registration: Current licensure in the State of Colorado as a Registered Nurse, or current active multistate Registered Nurse license. Current BLS certification through the American Heart Association or American Red Cross.

Education: Graduate of an accredited school of nursing. BSN preferred. Activity in professional organizations encouraged.

Experience: Six month of clinical practice in the clinical specialty of his/her unit and leadership/ management experience is required.

The Super Pool RN is responsible for the delivery of nursing care to the patient requiring telemetry, and/or medical surgical care. The Charge Nurse may rotate to areas other than his/her assigned service area/s, and will perform only those duties for which he/she has demonstrated competency. As a PACU Registered Nurse, you will play a critical role in the immediate post-operative care of surgical patients. Your responsibilities will include managing airway support, closely monitoring neurological and cardiac status, and responding promptly to any complications or side effects. As a Case Manager, your role will be to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans that reflect each patient’s unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care. 

Your responsibilities will include:

• Guiding patients and families through program orientation, explaining the rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights 
• Coordinating education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences 
• Developing and implementing individualized treatment plans that reflect the patient’s strengths, needs, and personal recovery goals 
• Completing psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team 
• Collaborating closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care
