Gestational Carrier Requirements

To ensure a safe and successful journey for both carrier and baby, applicants must meet the following criteria:

• Between 21–39 years old
• U.S. citizen living in a surrogate-friendly state
• Have delivered at least one child and are currently parenting
• No more than two (2) C-sections
• Not receiving government or public assistance (including Medicaid, WIC, SNAP/Food Stamps, or state-funded health insurance)
• No history of pregnancy complications (including gestational diabetes, pre-eclampsia, or pre-term labor)
• No history of schizophrenia, bipolar disorder, borderline personality disorder, or major depressive disorder
• Not currently taking medications unsafe for pregnancy (including certain mental health medications)
• Able to travel for medical screening, monitoring, and embryo transfer appointments
• Live a healthy lifestyle free of illicit or recreational drug use
• Have a stable home environment and strong support system

Compensation & Benefits

We recognize the commitment, time, and care involved in being a gestational carrier. Compensation includes:

Base Compensation

• $60,000 – $70,000(First-time carrier to experienced carrier)

Additional Benefits

• $250 monthly expense allowance
• $500 maternity clothing allowance
• $10,500 for multiple birth
• Up to $10,000 for loss of organs (per contract terms)
• Lost wages (carrier and spouse, if applicable — based on employment verification)
• Travel reimbursement
• $200 per week for housekeeping (if medically necessary)
• $20 per hour for childcare (if medically necessary)

All medical expenses, legal representation, and services related to the surrogacy journey are covered.

Summary

The Physician Assistant must practice in accordance with the rules adopted by the Ohio Medical Board, the Revised Code and institutional policies and procedures. The PA role is developed in relation to the specialty population of practice and reflects the scope and standards of their specialty, licensing board, and certifying organization (NCCPA). The PA is accountable to maintain competence through continuing education and review of practice. The PA roles include those of practitioner, researcher, consultant, and patient advocate for individuals, groups, and communities.

Nature and Scope

The PA should demonstrate skill in applying and modifying the principles, methods and techniques of medicine to provide ongoing patient care. The PA should have knowledge of the professional medical practice being able to give and evaluate patient care. The PA should be competent in taking medical history, assessing medical condition (s) of patients, and interpreting medical findings. The PA should possess the ability to maintain quality assurance and quality control standards. The PA should be able to implement effective communication skills with both patients and colleagues that demonstrate a commitment to consistently providing an excellent patient experience for every patient. The PA is expected to have capability of reacting calmly and effectively in emergency situations.

Qualifications

1. Applicant must hold a Master?s Degree from an accredited Physician Assistant program (MPAS, MHS, or MMS). 2. Applicant must have and maintain a PA certification from the National Commission on Certification of Physician?s Assistants (NCCPA). 3. Current Ohio licensure and ACLS certification is required. 4. Certificate to Prescribe must be current and maintained at all times. 5. A minimum of three years of related experience as a PA. 6. Excellent communication skills with both patients and colleagues are essential. 7. Computer literacy is required and must include Excel, Word and Outlook. Electronic Medical Records experience is an advantage, preferably EPIC. 8. Experience with, and a strong surgery background will be a great advantage.

The Registered Nurse (RN) is responsible for delivering safe, high-quality, patient-centered care utilizing the nursing process of assessment, planning, implementation, and evaluation. The RN provides direct patient care within the scope of professional nursing practice and ensures compliance with all regulatory, accreditation, and organizational standards.

• Deliver direct patient care to adult and geriatric populations.
  
• Monitor and interpret cardiac telemetry rhythms and respond to acute changes in condition.
  
• Administer medications, IV therapy, and treatments per physician orders.

• Initiate emergency interventions in accordance with BLS/ACLS standards.

EDUCATION, EXPERIENCE, TRAINING

1. Current and valid state RN License.
2. Current BLS certificate upon hire and maintain current.
3. Current ACLS certificate 30 days upon hire and maintain current preferred.
4. Previous Acute Care hospital experience preferred.
5. Ventilator Management experience preferred.
6. Bachelor of Science in Nursing (BSN) preferred.
