What You’ll Do:

  • Collaborate with multidisciplinary teams at hospitals and physician practices to offer recommendations and ensure optimal patient outcomes

 Requirements:

  • Bachelor’s Degree in Pharmacy
  • 2 years or more experience
  • Current MO Pharmacist License (RPh)
  • Medication Therapy Services (MTS) – Have currently or able to obtain

Experience:

  • 5 years of hospital pharmacy experience (highly preferred)
  • Experience in one of the following areas: Home Infusion Pharmacist; Home Infusion Clinical Pharmacist; Admissions Home Infusion Pharmacist

The pharmacy staff oversee cutting edge automation in medication distribution systems. Our pharmacists provide clinical services including participation in patient rounds, nutritional therapy services, screening patients daily for change in renal functions and adjusting the dosage accordingly. The pharmacists work in a team-structured environment with other healthcare professionals 24 hours a day.

Role Purpose

Evaluates and recommends appropriate drug therapy to achieve optimal cost-effective patient outcomes, by working directly with physicians, other health professionals, and patients. Participates in the development, provision and evaluation of comprehensive pharmaceutical care and quality assurance programs through scholarly activities. Serves as a resource to the pharmacy staff and other hospital departments.

 

Responsibilities

Assess the overall status of the patient’s health condition and evaluates medication profile for optimal patient outcomes. Monitors therapy and goals of care. Documents all activities and communication as appropriate.Participates in professional development and research activities.Contributes to performance improvement, clinical effectiveness, drug use evaluation, and HSO/BJC committee activities.Participates in the education and training of pharmacy staff, residents and students, and serves as a role model and mentor.Participates in the identification and implementation of targeted drug cost reduction programs.
