Part time RN Case Manager, Emergency Dept

20 hours/week

CARE COORDINATION:  The RN Case Manager works with the multidisciplinary team including physicians, staff, and payers to ensure patient's progress along the continuum of care in an efficient and cost-effective manner that ensures quality outcomes. The RN Case Manager acts as a patient advocate and communication link with other departments and community resources. Supports nursing staff in development of individualized, evidence-based, outcomes oriented, and safe practice

Responsible for patient assessment of complex clinical discharge needs and establishment of an appropriate plan of care with multidisciplinary team. Demonstrates critical thinking skills and oversees the care coordination of defined population. Ensures that patient goals are set with patient/family, negotiates healthcare resources and educates patient/family regarding available options of care throughout the continuum. Demonstrates clinical competence to care for a specific patient population. Facilitate patient education for safe and effective discharge.

PROFESSIONAL DEVELOPMENT: Demonstrates leadership behaviors that enhance and support team functions. Mentors and coaches staff. Maintains competencies and participates in continuing education for self and development of colleagues and students. Participates in delivering professional programs/ courses/in-services, internal and/or external, and community events. Seeks opportunities to promote staff education, certification, clinical ladder, and award programs. Maintains national certification, as applicable, and membership in professional organization(s). Creates opportunities to advocate for the role of the professional nurse as a key member of the interdisciplinary team.

POSITION REQUIREMENTS:

Minimum Education:   Required: Bachelor's Degree  

Preferred Education: Master's Degree

Minimum Experience: Required: 3-5 years of previous job-related hospital experience.

Preferred Experience: 6-10 years of previous job-related hospital and Inpatient Care Management experience

Preferred Certification: ACMA, CMGT-BC, CCM

Recent Case Management experience is preferred.

DETAILS: Recent clinical experience in medicine, surgery, critical care, telemetry, or ED is required. 

Compensation:

Salary Range: $35.89 - $62.91 per hour

MacNeal Hospital's Emergency Department is a Level II Trauma Center with two dedicated trauma resuscitation rooms in our 26-bed Emergency Department. We are Emergency Department Approved for Pediatrics (EDAP). We support an acuity-based staffing model in our fast-paced environment and provide care to a wide range of patients allowing our RN's to grow and gain valuable experience.

What Registered Nurse RN will need:  
•    A Degree from an accredited school/college of nursing is required - BSN or ADN
•    BLS Provider (accredited by the American Heart Association) required prior to hire
•    Illinois Board of Nursing license prior to hire

BSN Pay Range: $34.76 - $57.06 per hour

ADN Pay Range: $32.31 - $57.06 per hour

Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.

What you will do:

  • Patient Care and Assessment: Conducts thorough assessments of patients' health status, including vital signs, medical history, and current symptoms. Develops and implements individualized care plans based on these assessments.

  • Nurse Triage: Performs initial triage to prioritize patient care by evaluating the urgency of their conditions. Determines appropriate next steps for treatment or referral, ensuring timely and effective care.

  • Care Coordination and Communication: Collaborates with physicians, specialists, and other healthcare providers to coordinate care, ensure effective communication, and manage patient treatments and follow-ups.

  • Patient Education and Support: Provides education to patients and their families about managing health conditions, medication administration, and lifestyle changes. Offers emotional support and answers questions to help patients navigate their care.

  • Administrative and Documentation Duties: Maintains accurate and up-to-date patient records, including charting care provided, documenting patient interactions, and managing appointment scheduling and other administrative tasks.

What you will need:

  • A Diploma from an accredited school/college of nursing is required - BSN or ADN

  • BLS provider (accredited by the American Heart Association) required prior to hire

  • Current Registered Nurse License State of Illinois

  • 1-2 years of previous experience in a clinical setting.

Compensation:

Salary Range: $31.62 - $50.79/hr per hour

Education: Bachelor's degree in nursing required
