to join our post-acute care team. This position focuses on providing high-quality, patient-centered medical care to patients residing in skilled nursing and rehabilitation facilities. The ideal candidate will have experience managing medically complex, post-acute, and long-term care patients, with a strong commitment to continuity of care, interdisciplinary collaboration, and optimizing patient outcomes during transitions of care.
Responsibilities:
Provide comprehensive medical assessments and ongoing management for patients admitted to skilled nursing facilities for post-acute, rehabilitative, and long-term care.

Manage and coordinate treatment for patients with multiple chronic illnesses, acute conditions, and complex medication regimens.

Conduct daily or scheduled rounds in assigned SNFs, ensuring timely evaluation and management of acute changes in condition.

Collaborate closely with facility staff, nursing teams, rehabilitation specialists, social workers, and case managers to deliver coordinated, patient-centered care.

Communicate effectively with families, caregivers, and facility staff regarding patient goals, treatment plans, and care transitions.

Ensure compliance with all SNF regulations, documentation standards, and quality measures, including participation in care plan meetings and facility quality improvement initiatives.

Partner with hospitalists, primary care physicians, and specialists to support smooth transitions of care and reduce hospital readmissions.

Participate in quality improvement, population health, and value-based care initiatives related to post-acute and long-term care.

Qualifications
Medical degree (MD or DO) from an accredited school of medicine.

Or one full year of graduate-level education in a field of study related to law enforcement (e.g., criminal justice, homeland security, justice studies, law enforcement, courts and judicial systems, forensic technology, forensic psychology, or corrections and rehabilitation) from an accredited college or university;ย ORย 

A Combination of Experience and Education: This will be calculated using your resume and official or unofficial transcripts submitted with your application.ย

Our Team

- 8 DVMs that are collaborative, consult freely on cases, and orchestrate plans for pets across a variety of modalities offered at the practice. Multiple skilled in surgery, dental, nutrition, alternative modalities, and more.

- 4 RVTs with over 10 years of experience each, 4 more pursuing a license, our assistants have 5+ years of experience, and we work closely with US Davis vet students.

- Highly efficient, fully utilized, and extremely skilled technicians. Each doctor has 2-3 assistants as well as 3 for surgery days to support.

- Incredible referral relationships, including specialists that come in for cardiology, orthopedics, radiology, ultrasound, etc.

- We welcome the unique skills each new doctor brings, but we're especially excited about interests in orthopedic manipulation (chiropractic), ophthalmology, dermatology, ultrasound, or board-certified specialties. We also enjoyed mentoring an early-career doctor last year and would love to support someone interested in mentorship.
