The Clinical Social Worker is an integral member of the care team and contributes to the care coordination for patients with multiple social stressors and/or behavioral health concerns.  The Clinical Social Worker utilizes screening criteria for the coordination of quality health care services, reduction of service fragmentation, enhancement of quality of life, and the appropriate use of healthcare and community resources.

Role Description:

Core Responsibilities:

• Collaborate with the primary care teams to identify appropriate patients to refer to the Oak Street Behavioral Health Program

What are we looking for?

Required:

Position Summary:

The Senior Executive Assistant (EA) provides high-level administrative and organizational support to C-Suite executives at AdaptHealth. This role ensures seamless day-to-day operations, proactively manages executive priorities, and maintains the highest level of professionalism, confidentiality, and attention to detail. • Possess a current valid driver's license
