Be the Best Hospice Admissions RN You Can Be

If you meet these qualifications, we would love to meet you:

  • Licensed as a Registered Nurse in the state(s) of practice, with an associate degree preferred and CPR certification recommended (required for direct patient care in Georgia). Must meet state-specific regulatory requirements.
  • Previous nursing experience in hospice, home health, general medical-surgical, or population-specific care (e.g., pediatrics, geriatrics) is required, along with willingness to serve on-call.
  • Must have a reliable vehicle, valid driver’s license, and the minimum required liability auto insurance.
  • Strong communication and organizational skills, with working knowledge of Microsoft Office Suite and EMR systems. Must be able to work effectively as part of an interdisciplinary team.
  • Physically able to lift patients over 200 pounds (with assistance), stand, walk, sit, stoop, and perform other physical movements frequently. Regular exposure to indoor environments and moderate noise expected.

Responsibilities:

  • Conduct comprehensive assessments and collaborate with patients, families, physicians, and the interdisciplinary team to develop and adjust individualized plans of care
  • Coordinate and communicate care needs with internal and external partners, ensuring timely updates, interventions, and resource utilization

Pilot Program and Enhanced Recovery After Surgery Program Navigator in Danbury, CT (Fairfield County, 70 miles from NYC) Western Connecticut Medical Group is seeking a full-time Advanced Practice Practitioner (APP) APRN/PA for the Pre-Admission Testing and Enhanced Recovery After Surgery Programs to join our established and busy employed practice at Danbury Hospital in Danbury, Connecticut, (Fairfield County). We are located about 70 miles northeast of New York City and near the border of Westchester and Putnam Counties in NY. This is a newly created position for a growing department to work with about 30 physicians in the Anesthesia department and 2 APRNs. We offer a highly competitive compensation and benefits package, including allowances for CMEs and Dues expenses. The ideal candidate will be a board-certified Advanced Practice Practitioner (APRN/PA) with at least two years? experience in either primary care/internal medicine, pain management or surgery. The position is Monday-Friday. No weekends, no holidays, or call. Job Summary: Provides medical care for patients and determines risk stratification pre-operatively in the pre-admission testing setting. The APP provides preoperative evaluation and management to patients. The APP Coordinates the clinical and administrative activities related to the office to ensure the delivery of quality and cost-effective patient care. The Advanced Practice Practitioner reports to the Clinical Director of the Pre-Admission Testing/Gibson Perioperative Surgical Home and Anesthesia Pain Service. (GPS/PAT/APS). Job Overview

A respected, multi-state health network is seeking a Full-Time Night Physician Assistant (PA) Hospitalist to join a collegial team at a prominent 371-bed community hospital and Level II Trauma Center. This position is ideal for an experienced or new graduate PA who thrives in an inpatient setting, providing primary medical care to hospitalized patients during variable night shifts (12 hours). The role offers the chance to work independently alongside experienced Hospitalist Physicians, contributing to a vibrant teaching environment located in a desirable suburban area.

Key Responsibilities

  • Primary Patient Care: Under the supervision of a physician, provide primary medical care to hospitalized patients.
  • Clinical Management: Assist patients in their recovery from acute illnesses and work to minimize the effects of their chronic illnesses.
  • Collaboration: Work in a collegial environment alongside experienced physicians and PAs in the Hospitalist Medicine division.
  • Schedule: Must be able to work a variable block night schedule, typically 8 PM ? 8 AM (12-hour shifts), amounting to approximately 14 shifts per month, including two weekends per month.
  • Skill Growth: Utilize opportunities within the network to grow personal clinical skills and expertise.

Requirements

  • Education: Bachelor?s degree and graduate of an approved Physician Assistant (PA) school.
  • Certification: Certification by the National Commission on Certification of Physician Assistants (NCCPA).
  • Licensure: State PA license, DEA, and state controlled substance registration are required.
  • Life Support: Current ACLS & BLS certification is mandatory.
  • CME: Must maintain 50 CME hours over two years.
  • Skills: Strong interpersonal and communication skills.

Preferred Qualifications

  • Prior PA Hospitalist experience is preferred.
  • New graduates with inpatient Internal Medicine experience are encouraged to apply.
