Role Description:
The purpose of a Nurse Practitioner at Oak Street Health is to provide effective and equitable value-based primary care to adults on medicare to keep them happy, healthy, and out of the hospital. Nurse Practitioners see patients independently and collaborate with physicians, depending on the patient’s specific situation. Nurse Practitioners provide longitudinal care and build meaningful patient relationships; this creates trust and helps our patients understand and feel connected to their care plan.
Nurse Practitioners practice in our outpatient practices on a collaborative care team composed of a physician, nurse practitioner or physician assistant, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers. We partner with a network of elite specialists and hospitals for specialty and acute care. As such, our healthcare providers can focus on care within the clinic: primary care, care coordination, and population health.
Core Responsibilities:
- Provision of exceptional primary care.
- Conduct office visits for routine and acute issues.
- Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation.
- Care coordination with other providers, specialists, testing facilities, and agencies.
- Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years)
- Assisting the care team with phone triage and outreach.

Required Qualifications:
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification in at least one of the following specialties:

Apprehending, physically restraining, or working with law enforcement officials to identify or apprehend violators of state, federal, or immigration laws.
Proficient in the use of firearms, preparing investigative reports, serving court orders (e.g., warrants, subpoenas, etc.), and gathering evidence for criminal cases prosecuted through the court system.

Description:
JOB PURPOSE: Serves as a member of a surgical service line team by assisting in management of post-operative and critically ill patients in the hospital, as well as evaluating pre and post-operative patients in the office setting. Applies specialized skills and knowledge related to the care of patients in the CVOR, ICU, CCU and CVICU areas. Contributes to excellence in patient care, research, teaching, consulting and provides leadership to the organization.
KEY RESPONSIBILITIES: 1. Provides direct clinical services for pre-operative and post-operative patients, within institutional guidelines and utilizing evidence-based clinical practice standards under the supervision of the patients physician. 2. Obtains patient health histories and complete physical examinations and documents findings in accordance with hospital requirements. 3. Orders appropriate diagnostic and laboratory tests, explaining the necessity, preparation, nature and anticipated effects of procedure(s) to patients, patients family, staff and healthcare learners. 4. Performs approved therapeutic or diagnostic procedures based upon patients clinical status and documents procedures in accordance with hospital requirements, including obtaining arterial, venous, and capillary blood samples, urine samples, sputum samples and cultures; performing cardiac output measurements, other hemodynamic measurements and EKGs; assisting with the performance of radiographic studies; assisting physician with diagnostic and therapeutic procedures; etc. 5. Performs cardiopulmonary resuscitation and external cardiac massage; airway management including bag and mask ventilation, endotracheal intubation and extubation, changing, positioning and repositioning endotracheal tubes; administration of life supporting drugs according to established protocols; and defibrillation according to Advanced Cardiac Life Support standards. 6. Under physician direction and within the credentialing standards of the Hospital, manages arterial catheters, central venous catheters, pulmonary artery catheters, temporary pacemakers, intra-aortic balloon pump catheters and chest tube placement, management, and removal. 7. Interfaces patients with monitoring and instrumentation equipment, and directs and educates others in performing the same functions. 8. Formulates and implements a treatment plan for acute illness management in collaboration with the designated primary supervising physician and/or attending back-up physician(s) and documents in medical records and appropriate narrative summaries. 9. Assesses patients for changes in status and institutes appropriate interventions, including initiation of necessary emergency interventions to stabilize patient. 10. As part of our team, you will have the chance to provide top-notch medical care to our Nation's Veterans in a fast-paced environment.
Responsibilities:
- Provide quality medical care in the Emergency Department
- Work 10-hour shifts with a fast track attached to the ED
- Manage an annual ED volume of 24,000 patients
- Experience an admissions rate of approximately 17%-20%
Qualifications:
- American Academy of Nurse Practitioners Certification Board (ANCC)
- State Nurse Practitioner License
- Minimum of 2 years of NP experience with at least 1 year in Emergency Medicine
- Active BLS and ACLS Certifications
- Active DEA Registration

Description: Advanced Practice Practitioner
- Primary Care Outpatient Piedmont Healthcare is recruiting for an Advanced Practice Practitioner to join the Primary Care team at our Outpatient Practice.
The position is full time, Monday through Friday.
In this role you will: Provide medical services to individuals, families and/or groups.
Emphasize health promotion as well as acute and chronic disease management.
Augment a physicians ability to provide medical services to patients.
Collect and document data, conduct diagnostic and therapeutic procedures, order and schedules laboratory studies and professional consultations and provide direct patient care services.
The APP will work collaboratively with multiple IM providers in the care of the PCPs patient panel.

New Extended Hours:
- Weekday Evenings: 5:00 PM ? 9:00 PM
- Weekends: 12:00 PM ? 6:00 PM
These hours allow families to avoid unnecessary ER visits and enable parents to schedule well-child visits without missing work.
