Primary Vascular Care is looking for a Nurse Practitioner (NP) or Physician Assistant (PA) to perform on-site clinical assessments to determine appropriate patient treatment plans of residents of client facilities. This position reports to the Lead Nurse Practitioner.
The NP/PA will work in a mobile practice, primarily serving a long-term care setting.
Must be able to travel to Queens and the Bronx.
ESSENTIAL FUNCTIONS:
Performs on-site screening clinical assessments to determine appropriate patient treatment plans of residents of client facilities by the completion of a comprehensive assessment form
Functions independently to perform appropriate history and physical for complex acute, critical, and chronically ill perioperative patients
Orders and reviews diagnostic and therapeutic tests relative to patient’s age-specific needs
Assists with the provision of care in accordance with facility, state, and federal regulations
Monitors the effectiveness of interventions
Maintains relationships with all appropriate contacts in facilities
Educates family members, clients, and prospective accounts regarding services and programs offered by Primary Vascular Care
Assists with the development and maintenance of physician relations, as assigned
Completes weekly, monthly, and other reports, as assigned, in a timely and comprehensive manner
QUALIFICATIONS:
Bachelor’s Degree required
Degree from an accredited Nurse Practitioner or Physician Assistant Program required
Current NP License required
BLS – Basic Life Support through American Heart Association required
ACLS – Advanced Cardiac Life Support through American Heart Association preferred
Current Infection Control Certification required
Board Certified/Board Eligible required
Physical, Vaccines and Titers: PPD/TB, Hep B, MMR, Varicella required
COVID and Flu Vaccine preferred
1 year of NP experience preferred
Reliable transportation (personal vehicle) required

* Prescribe medications and formulate ongoing treatment and disease management plans.
* Recommend suitable management for the patient, including preventive health recommendations and habits.
* Provide relationship-centered care, considering the patient's psychosocial and physical needs.
* Maintain complete and accurate medical records.
* Communicate effectively with the clinical team to ensure the total delivery of quality care.
* Participate in medical staff educational programs and meetings.
Qualifications:
Education & Experience:
* Doctor of Medicine (MD) or foreign equivalent.
* Completion of a four-year postgraduate medical training from an accredited residency program in the area of practice and three years of cardiology fellowship training.
* Board Eligible/Board Certified in Cardiology Medicine, Nuclear Cardiology, or Echocardiography with a preference for Level II or Level III COCATS certification.
* Seeking both experienced and newly trained providers.
* Current, unrestricted New York State license.

Job Summary:
We are seeking a highly motivated and experienced Nurse Practitioner (NP) Supervisor to lead our team of Nurse Practitioners. The ideal candidate will be responsible for the day-to-day oversight of NP operations, ensuring the delivery of high-quality, evidence-based patient care while fostering a collaborative and supportive work environment. This is an excellent opportunity for a highly skilled PA with cardiac surgery and operating room experience to work alongside a respected cardiovascular surgical team in a high-acuity clinical environment.
Position Overview
The Cardiothoracic Surgery Physician Assistant will play a key role in supporting cardiovascular surgical services and managing patient care across the perioperative continuum. The PA will collaborate closely with attending surgeons, medical teams, and clinical staff while also providing leadership and mentorship within the PA team.
This position requires strong clinical judgment, surgical assisting experience, and the ability to provide high-level patient care in a fast-paced hospital environment.
