The RN Navigator in Population Health is responsible for coordinating and managing patient care across the healthcare continuum. The Specialty Clinical Pharmacist is responsible and accountable for the provision of safe, effective, and prompt medication therapy. Specialty Clinical Pharmacists provide proficient direct patient-centered care in a decentralized practice setting.
