Job Summary

Administers the nursing program in a long term care facility to maintain standards of resident care. Facilitates the optimization of the geriatric care process to improve the quality and efficiency of service. Responsible for the overall quality of care provided by the organization's nursing personnel. Advises medical staff, department heads, and administrators in matters related to nursing service and strategies. This role has a larger size, scope, or complexity. Interprets policies and objectives of nursing service to staff and community groups. Monitors the operations of the nursing staff and ensures compliance with regulations on organizational and governmental standards and practices. Schedules staff and conducts employee performance reviews. Provides nursing care to residents on an as needed basis. Has overall responsibility for the day-to-day operations of long term care facility. Includes quality of resident care, program/care management, environmental safety, budget constraints and responding to identified needs to ensure regulatory compliance. Maintains communication and addresses the rapidly changing healthcare environment in which clinical services are delivered across the system. Qualifications

Bachelor's degree in nursing required.

Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).

For Rural Health Network facilities and Good Samaritan Society (GSS) locations only, employees who do not possess a Bachelor’s degree, leadership may consider an educational plan with proven continuous action toward achieving a Bachelor’s degree within five years of hire into position. Five years nursing experience required. Housing with Services, housing and urban development (HUD), and/or assisted living experience also helpful. Thorough knowledge of the nursing practice act and standards of nursing care, and a working knowledge of administrative and management techniques.

Job Summary

The Nursing Assistant (NA) serves as caregiver to the resident during the scheduled work period in long term care. Provides resident-centered nursing care and daily living assistance to assigned resident under the supervision of a registered nurse (RN). Knowledge of and delivers age-appropriate care related to the physical and psychological needs of the resident as per care plan. Considered a member of the nursing team and is expected to know, and will be held accountable for, following infection prevention and control policies and Personal Protective Equipment use. Provides assistance with basic health care needs including daily living activities that may include, but are not limited to, bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psychosocial support and other personal care to assigned resident. Assists the resident in transferring, repositioning, and walking using correct and appropriate transfer techniques and equipment and also provides range of motion and passive exercises. Completes tasks related to food preparation and dining, while creating a positive dining experience for the resident. Assists residents with meals and snacks, provides water and supplements as appropriate.

The NA aids providers and nursing staff members with procedures, if needed. Documents resident interactions as needed and the outcomes related to resident care, behavior, activity, and dietary. Communicates resident's changing condition and care related concerns/responses to the charge nurse. Collects non-invasive body fluid specimens and gathers vital signs. Provides housekeeping and laundry services to assist residents with keeping living areas clean and operational. Provides assistance and documentation with wellness and recreational programs and encourages residents to participate. May participate in departmental/organization committees and task forces as needed.

Qualifications

High school graduate or general equivalency diploma (GED) preferred, but not required.

Minimum of 18 years of age.

Prior clinical or nursing assistant experience in long-term care preferred.

Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state certified CNA within four months of date into the position by being enrolled in a training program or in the process of obtaining certification through Endorsement. CNA training and certification required and provided by facility.

Colorado: Depending on facility, Qualified Medication Administration Person (QMAP) certification with the Colorado Department of Public Health & Environment required.

Satisfactorily completes yearly department specific competency testing and maintains a record of continuing education credits for each year. A minimum of 12 hours continuing education are required annually. Maintains all department specific required certifications.

When applicable, for applicable states, certified and registered with the Unlicensed Assistive Person (UAP) Registry.

Dependent on facility and required responsibilities, this role may classify as a Sanford Category II Driver requiring a motor vehicle report and proof of valid driver’s license. Also must be medically qualified to perform the essential driving functions of this position as per our Sanford Driving Policy per the leaders request.

Job Summary

The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician. Demonstrates knowledge of nursing principles for the resident in the relevant clinical setting. Qualifications

Responsible for administration of nuclear imaging procedures, calculation, diagnostic, therapeutic radiopharmaceuticals, and adjunctive medications using a variety of equipment and strictly adhering to relevant safety standards. Performs quality nuclear medicine imaging examinations through the application of advanced technical knowledge while maintaining strict attention to patient care in accordance with department policy and procedure. Carry out hot lab duties and perform a variety of nuclear medicine studies on inpatients and outpatients in all age groups from neonate to geriatric. Have an understanding of nuclear regulatory commission (NRC) rules and regulations, practice radiation safety, safely operate all cameras and equipment in the department, process and send data to required destinations via computer. Understands the theories and principles behind the equipment and tests performed in the nuclear medicine department.

Prepares, measures, and administers radiopharmaceuticals and therapeutically dosages under the direction of a physician. Employs radiation-detecting equipment to perform patient imaging procedures; describes procedures to patients and answers questions. Implements radiation safety standards and quality assurance into daily work and monitors a patient's condition during the course of an imaging procedure. Collects patient information, analyzes data and provides quality images for a physician to diagnose a patient.

Dependent on needs of facility and department may also be responsible and cross-trained in performing positron emission tomography computed tomography (PETCT) scans with thorough understanding of all processes related to PETCT. Has ability to perform nuclear medicine procedures after appropriate orientation to the department. Able to work with growth and development needs of unique client populations. Confirmation of thorough understanding of all processes related to PETCT.

Qualifications

Must have completed an educational program and clinical experience accepted by the American Registry of Radiologic Technologists or completed a formal AMA accredited Nuclear Medicine program and be certified by the CNMT or NMTBC.

One year experience as a staff radiologic technologist; if graduate of a formal Nuclear Medicine program, no experience is

required. Must have a thorough understanding of diagnostic radiographic principles, including film processing, quality control

and radiation protection.

Must be registered by the American Registry of Radiologic Technologists (ARRT) or be a Certified Nuclear Medicine Technologist (CNMT).

Job Summary

A degree in Respiratory Care from an accredited respiratory care program is required; minimum associate degree or equivalent as defined by the National Board for Respiratory Care (NBRC).
