At Wegmans, our Project Controls Manager is responsible for providing subject matter expertise while meeting required specifications and expectations. This role will support the development department by optimizing life cycle strategies and preventative maintenance plans to ensure the highest quality work and equipment reliability.

What You'll Do:

- Manage and develop a team of employees, engage in meaningful conversations, build trusted relationships and provide the resources, feedback, and development opportunities they need to be successful
- Develop and implement standards for category critical systems & equipment; approve system modifications
- Utilize subject matter expertise to evaluate the quality of work performed by contractors and technicians and provide feedback
- Leads equipment & process optimization efforts; responsible for change management of systems and assisting with troubleshooting
- Maintain system capacities; keep current and accurate data of system operation
- Deliver concise, data‐driven project updates and results to senior leadership
- Establish partnerships within Construction, Design Services, Indirect Procurement, and Maintenance Operations on engineering standards for systems and equipment
- Establish strong working relationships with Division Store Maintenance Managers, Manufacturing and Facility Maintenance Managers, contractors and Division & Store Managers to develop clear expectations, prioritize and mediate project challenges
- Support training for team members on category equipment and systems
- Use data & costs to develop and own category preventative maintenance program & compliance standards
- Set strategy and goals for category; participate in capital planning and represent category on projects
- Review and analyze category asset and repair history; identify root causes, and provide recommendations for continuous improvement
- Utilize knowledge of Wegmans BIM strategy to help implement throughout the Maintenance organization

Requirements:

- 5 or more years of experience in Building Management Controls Systems or Industrial Control Systems
- Project management experience including planning, executing and closing projects
- Experience presenting project updates and outcomes to senior leadership
- 2 or more years of experience managing and developing others
- Experience with one or more control systems for commercial refrigeration, industrial refrigeration, manufacturing or building automation systems
- Advanced knowledge of technical systems and equipment
- Ability to take complex data and translate into actionable recommendations

Preferred Qualifications:

- Bachelor's degree in a related field
- Experience with maintenance building management and control systems and/or programs, i.e.: Delta, BMS, Emerson-CPC, or Allen Bradley PLC
- Experience building and maintaining external vendor relationships
- Experience developing maintenance programs
- Experience leading divisional or companywide initiatives or projects
- Experience utilizing CMMS Systems

Mount Sinai Health System is seeking a Senior Medical Director of Quality and Patient Engagement.

The Senior Medical Director (SMD) for Population Health Quality and Patient Engagement drives the implementationand ongoing improvement initiatives that enable Mount Sinai to deliver better value to its patients, its customers (i.e.,plan sponsors and payers), its providers, and its partners. At Mount Sinai, value is defined broadly and encompassesimproved health outcomes, more efficient operations, better patient experience, more joyful care team environment,and improved financial performance in value-based contracts.

MSHP seeks a Medical Director (MD), Physician Engagement and CDQI, who will (1) be responsible for driving key population health goals including access, quality, utilization and accurate risk adjustment with a group of practices and (2) close partnership with the CDQI Director to drive accurate risk adjustment throughout the system. This Medical Director must be effective at influencing physicians, medical director and other key practice staff to drive value based care success. He/she will support the establishing and hardwiring processes to drive excellence in efficiency of care, quality and accurate risk adjustment for all populations served, especially our patients covered under a Medicare arrangement (MSSP or MA). The MD will report to Vice President, Clinical Integration and Population Health with shared accountability to Director, CDQI.

SMD

The primary focus of the SMD will be:

  • Select Employed Ambulatory Metrics
  • Select Voluntary Ambulatory Metrics
  • Epic Optimization to Support Above
  • Primary care patient engagement rates
    • A mission critical metric for care excellence and primary care success)
    • Focused on all payer classes, with deeper focus on patients who have Medicare and Commercial insuran

This SMD will report to the CMO, Population Health and Clinically Integrated Network. The SMD will partner with the

MSHP analytics team, Digital and Technology Partners (DTP, i.e. IT) Epic EMR team to drive targeted quality and patientengagement performance most relevant to Mount Sinai's value based care contracts and select system quality goals(defined with the CMO of Population Health and CIN, and other key system stakeholders.

The ideal candidate will have significant experience (5+ years) in leading quality initiatives in the ambulatory setting, and

a record of accomplishment in meeting or exceeding quality metrics. Leadership experience in ACO or value-based carequality metrics is required. Key personal attributes include a positive and enthusiastic attitude, efficient and focus onkey results, and a balanced mix of confidence and humility.

Key Expectations also include:

  • Co-develop strategy with team and CMO, Population Health and CIN,
  • Identify and track performance on key indicators,
  • Analyze performance against national benchmarks,
  • Plan and co-develop content for physician education/engagement initiatives

Qualifications

  • Board-certified and in good standing with a minimum of 3 years clinical experience and at least 3 years of
  • demonstrated administrative and leadership responsibilities

Responsibilities:

The Physical Therapist, School Based serves students with various physical, behavioral, and developmental challenges. Ages range from 3-21 years old. Age specific experience and/or special training and/or expertise is required to serve this population.
RESPONSIBILITIES/ACCOUNTABILITIES:
Implements treatment plans and programs through consistent daily and weekly schedules that focus on functional and measurable outcomes to be shared with the school and/or supervisors.
Delivers specialized individual and group physical therapy instruction as mandated in students' Individualized Education Plans (IEPs).
Collects data and completes reports regarding the student's goals and progress.
Consults with other professionals (Teachers, SLP, OT, PCP, etc) on the student's treatment team to ensure a cohesive and comprehensive therapy team.
Assesses and documents each student's progress through formal and informal tests and measurements.
Writes and prepares annual review reports and goals for each student in accordance with their IEP dates and requirements in order to remain in compliance with state guidelines.
Provides physical therapy expertise when developing FBA/BIP.
Provides parent and caregiver support and education to discuss the student's progress and strategies for continued support outside of school.
Attends all school related meetings as scheduled, such as, but not limited to, Triennials, Annual Reviews, Domain, Referrals, and pre-IEP meetings for each individual student.
Ensures a suitable learning environment by setting up and preparing materials and supplies as needed.
Supervises PTA(s) in accordance with state practice act guidelines, as needed.
Communicates and follows up with schools and families to obtain appropriate physician signatures/orders for school-based physical therapy referrals in order to initiate and continue with services.
Follows the safety guidelines and directives for the student in regards to functional mobility and orientation (i.e. transfers, wheelchair or walker mobility, etc.) and exercises provided by other related service providers (i.e. occupational therapist, vision itinerant, hearing itinerant, speech and language pathologist, orientation and mobility instructor, etc.).
Utilizes communication devices and assistive technology as outlined by the speech language pathologist and itinerants.
Follows any fire drill or safety guidelines outlined for each individual student.
Provides screenings and evaluations to determine requirements for skilled physical therapy intervention relative to student/client's level of function. This includes, but is not limited to the following areas: school function, range of motion, strength, positioning, contracture management, conducting home assessments, discharge planning, development of maintenance programs and other related client care issues.
Provides documentation to maintain the quality of therapy records, as it relates to reimbursement guidelines, accuracy of billing information, and school requirements.
Participates in school trainings, in-service, and professional development opportunities.
Performs other related duties as required.
