Clinical Coordinator - IDD Services

- Ensure that support plans and services adhere to Individual Plans, quality standards, and regulatory requirements.
- Complete structured assessment interviews with parents/care providers.
- Conduct observation of the individual in appropriate settings such as home, school, or community.
- Review social history information, complete functional assessments, and target appropriate behavior baseline information for each referred individual.
- Write and submit a summary of assessment and recommended behavior support plan components to interdisciplinary team (IDT) or operations leader, as applicable.
- Advocate for the human and civil rights of individuals receiving services from the agency by attending and presenting behavior support plan information to review committees.
- Document progress and activity; review records and logs to stay abreast of changes in service plans; maintain confidentiality; complete billing documentation as applicable; organize and record all documentation in an accurate and timely manner.
- Maintain healthy and professional relationships with individuals, friends, families, guardians, and case managers; implement the company’s Customer Service Standards.
- Report any instance of alleged abuse or neglect according to internal and external standards; report medical, behavioral, and other incidents following company policy and external requirements.
- Maintain confidentiality and respect the rights of individuals according to applicable bill of rights; practice universal precautions; assist individuals in exercising their rights.
- Support and train staff in implementing Individual Support Plans (i.e.: behavior intervention plans) and conduct classes and orientations as assigned. Attend staff meetings and interdisciplinary team meetings as needed.
- Assist with socialization and behavioral development, personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other activities of daily living as needed.
- May accompany individuals to medical appointments; relay instructions and information to and from medical providers as required.
- If assigned, accurately administer and document delivery of medications and treatments; promptly report administration errors; maintain appropriate security of controlled medications and other medications and supplies.
- If assigned, monitor individual’s health, documenting concerns and communicating with nurse or supervisor as appropriate; follow individual health care directives.
- May transport individuals into the community; drive safely and according to local laws; assure proper use of safety equipment including seat belts, lifts, and wheelchair ties; report accidents and safety concerns to appropriate authorities, supervisor, or maintenance personnel immediately.
- Check water temperature as required when assisting with bathing; participate in safety drills and protect persons being served in the event of emergency.
- Comply with all established safety policies, procedures, and rules; report unsafe hazards to supervisor and participate in safety-related training or activities.
Qualifications:
- Bachelor’s Degree in a human services field.
- Two years of related experience.
- Training in behavior modification techniques and/or experience providing behavior management treatment as required by state or program funder.
- Knowledge of specialized populations such as individuals with a developmental disability, brain injury, or mental health; expertise in special disciplines such as behavioral support, early intervention, or crisis intervention.

SUMMARY
The Executive Director provides market leadership and direction for the effective management of operations and the delivery of home health agency services, strategic planning, development and attainment of market/Company goals and objectives.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
Employee must have regular attendance/punctuality, be able to work with others at all levels of the Company, have exceptional customer service, and be completely honest. Other assigned duties include:
- Provides leadership and executes a strategic planning process that supports market growth including census, staffing, retention, utilization, and compliance.
- Oversees and manages annual operating budget.
- Ensures that the performance and operations are in compliance with all regulatory requirements including all other local, state, and federal regulations related to home-based care.
- Assures recruitment and retention of clinical staff adequate to meet patient needs and guarantee prompt and timely admissions of all referrals.
- Maintains a system of staffing which is based on patient needs and which defines the number and mix of Clinical Staff and office support staff needed.
- Works with the sales team to develop and execute growth strategy.
- Reports monthly KPIs to leadership team, creating action plans to address areas of opportunity/deficiency.
- Demonstrates and maintains a working knowledge of and ensures Agency management adheres to the Medicare Conditions of Participation and State Operations Manual
- Directly and indirectly supervises market staff.
- Demonstrates and gives proper assistance in handling of satisfactory on-site surveys by all regulatory bodies as pertains to home health.
- Participates in the development of the company-wide Performance Improvement Plan in consultation with Executive Team and Clinical Coordinators.
- Establishes a clear channel of communication between Executive Team, Clinical Coordinators and staff to keep them informed and promotes professional working relationships with alt departments and agencies.
- Demonstrates understanding of the principles of human resources, applicable laws, policies and procedures.
- Leads and attends company committees as requested.
- Exercises authority through channels to ensure delegation and empowerment of staff.

SUPERVISORY RESPONSIBILITIES:

EDUCATION and/or EXPERIENCE
Required:
- Bachelor’s degree in a related field
- Must be at least 21 years old
- At least eight (8) years of experience in healthcare; or equivalent combination of education and experience.
- At least two (2) years of experience in a leadership role in healthcare
Preferred:
- Master’s degree in a related field desirable but not required.
- At least two (2) years of experience in a leadership or supervisory role in the home healthcare strongly preferred
- Department of Labor / EEOICP experience strongly preferred
Travel Requirements: Frequent travel within local market required.

- Experience designing and integrating hydronic systems including pumps, piping networks, valves, and heat exchangers for data center cooling applications.

We are seeking a highly skilled, technically proficient and detail-oriented Principal Mechanical Engineer with expertise in metal structures, frame design, pipes and fabrication to support the development of power and cooling infrastructure for data centers. This role is critical in designing robust mechanical systems that house and support electrical and thermal management equipment, ensuring structural integrity, manufacturability, and compliance with industry standards.
What will you do?
- Experience designing and integrating hydronic systems including pumps, piping networks, valves, and heat exchangers for data center cooling applications.
- Knowledge of fluid dynamics, pressure drop calculations, and pipe sizing for chilled water and glycol-based systems.
- Understanding of installation constraints, maintenance access, and serviceability in large-scale mechanical infrastructure.
- Design and develop mechanical frames, enclosures, and support structures for data center power and cooling products (UPS systems, battery racks, CRAC units, containment systems, LVS, MVS).
- Perform advanced structural analysis using FEA tools to validate mechanical integrity under static, dynamic, seismic, and thermal loads.
- Analyze load paths, stress concentrations, and fatigue life of structural components to ensure long-term reliability.
- Design metal enclosures with considerations for airflow, thermal containment, EMI shielding, and environmental protection.
- Evaluate and implement new materials, coatings, and fabrication technologies to enhance product performance and reliability.
- Participate in customer engagements and site visits to understand deployment constraints and gather feedback for design improvements.
- Create detailed CAD models and fabrication drawings, including weldments, sheet metal, and machined parts
- Select appropriate materials and fabrication methods to optimize cost, performance, and reliability
- Collaborate with electrical, thermal, and systems engineering teams to integrate mechanical designs with functional components.
- Lead mechanical design reviews and contribute to cross-functional design validation efforts.
- Develop and maintain design documentation including BOMs, specifications, test plans, and engineering change orders (ECOs).
- Drive continuous improvement in mechanical design for manufacturability (DFM), cost reduction, and performance optimization.
- Interface with suppliers and customers to resolve fabrication issues and ensure quality standards are met.
- Support lifecycle management of mechanical components, including obsolescence planning and alternate sourcing.
- Conduct root cause analysis and corrective actions for mechanical failures or field issues.
- Ensure mechanical designs support airflow management, thermal containment, and integration with cooling systems (in-row cooling, rear door heat exchangers).
- Collaborate with compliance and certification teams to ensure mechanical designs meet global regulatory requirements (UL, CE, RoHS, REACH)
- May perform other duties and responsibilities as assigned

- Bachelor’s Degree in Mechanical Engineering or related engineering discipline.
Experience:
- 8+ years of experience in mechanical design and fabrication, preferably in data center, industrial, or power systems environment.
- Proficiency in CAD tools (SolidWorks, Creo, Inventor) and FEA software (ANSYS, Abaqus).
- Strong knowledge of metal fabrication processes including welding, sheet metal forming, and machining is a must
- Experience with structural design for heavy equipment or large assemblies, and familiarity with thermal and airflow considerations in enclosure design is required
