Position Summary
In this role, you will be responsible for providing direct nursing care for residents as well as general consultation regarding health-related concerns within the community.
Essential Functions, Key Duties, & Responsibilities
- Direct the daily activities of the certified nursing assistants and resident care associates in accordance with current federal, state and local regulations and established company policies and procedures.
- Follow the nursing model to promote holistic care for the residents.
- Ensure that policies and procedures are complied with by nursing personnel assigned.
- Participate in the admission, discharge and transfers of residents as required.
- Assess for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly.
- Transcribe physician orders to medical record and carry out orders as written.
- Prepare and administer medications as per physicians’ orders and observe for adverse effects as indicated.
- Perform wound treatments per physicians’ orders, observe for changes and documents accordingly as indicated.
- Schedule follow up appointments for residents and transportation needs as indicated.
- Provide nursing leadership to nursing personnel assigned to the unit.
- Provide direct care skills such as colostomy changes, tube feedings, wound care, suctioning, IV administration, etc. in accordance with current policies and procedures.
- Perform rounds to ensure resident needs are being met and personnel are performing their assigned duties.
- Collaborate with other members of the interdisciplinary team as needed to ensure residents’ needs are holistically met.
- Report any incidents or unusual occurrences to the community’s leadership team.
- Ensure that there is adequate stock of medications, supplies, equipment and notify appropriate personnel of needs.
- Initiate, review and update care plans as required.
- Participate in the survey process when inspection surveys arise.
- May participate in QAPI activities as needed or assigned.
- Ensure that infection control principles are followed and help correct deficiencies in practice as noted.
- Perform other duties as assigned.
- Comply with all policies and procedures and any updates.
Position Requirements
Education: A nursing degree from an accredited college or university
Licenses/Certifications: Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state

Summary
Corrections professionals who foster a humane and secure environment and ensure public safety by preparing individuals for successful reentry into our communities.
Duties
Participates in the administration, scoring, and interpretation of a wide variety of personality evaluation methods and is responsible for the preparation of comprehensive psychological reports for various purposes. Assists in interdisciplinary classification of inmates by identifying behavioral, emotional, organic, drug/alcohol, or personality problems and the subsequent development of appropriate programs on an individual basis. Completes reports for institutional staff on inmates who may manifest sudden bizarre behavior or thoughts, insomnia, psychosomatic complaints, suicidal preoccupations, depression, somnambulism, anxiety, situational crisis, or long-term disciplinary and segregation cases, etc. Conducts individual treatment and crisis intervention sessions on an as needed or emergency basis. Responsible for providing assessment and referral services for staff referred to psychology services by their supervisors or for staff personally requesting assistance with problems of alcohol use, drug use, or emotional problems which are affecting job performance. Along with all other correctional institution employees, incumbent is charged with responsibility for maintaining security of the institution. The staffs' correctional responsibilities precede all others required by this position and are performed on a regular and recurring basis.

The Opportunity
- Act as the operational administrator for the site Vituity leadership and as the interface for the practice to the hospital and community.
- Act as the front-line liaison for the provider team with hospital C-Suite, nursing leadership, nursing staff, and Vituity support team.
- Provide executive support to the site medical director and site management team to meet contract expectations.
- Provide support for all site financials to include, but not limited to, contract stipends, expense reimbursements, and site payroll timecards.
- Act as the super user for all Vituity software applications and as a point person for hospital software and hardware systems.
- As appropriate to site practice, provide support to Vituity providers acting as a percipient witness in criminal or civil disputes including, but not limited to, receiving and routing subpoenas, scheduling depositions and trial testimony as applicable, development of a provider fee schedule, and including malpractice carrier as appropriate.
- Provide office management to include, but not limited to, all aspects of meeting management, office systems, supplies, site events, and customer service.
- As applicable to the practice line, facilitate all aspects of the daily patient census and attend daily multi-disciplinary rounds.
- As applicable to the site practice, responsible for all aspects of the site clinical schedules to ensure adequate coverage with no disruption to patient care.
- Collect, track, and analyze all site financial and operational data.
- Project management as needed of the site operational programs to include, but not limited to, Operations Meetings, Patient Experience Program, Quality/Performance Improvement Program, Advanced Provider and/or Scribe Programs, and Student or Resident rotations.
- Ensure all aspects of recruiting and on-boarding are completed for new providers and employees as well as locums and reservists and/or residents and students.
- Ensure all licensed providers complete their recredentialing timely and appropriately for their licensing, certificates, and credentials required by Vituity and hospital Medical Staff Office.
- Ensure all site partners and employees remain in compliance with Vituity and hospital programs and other mandated training or requests.
- Act as the point person for all Vituity People Operations (Human Resources) relations to include, but not limited to, ensuring accurate site roster and compliance with employment law and Vituity policies.
- Develop and maintain site orientation checklists and manuals.
- Collaborate with Medical Director and Site Management Team in developing and maintaining site practice policies and procedures.
- As applicable to practice line, responsible for all medical records and data submission to Revenue Cycle Management in a timely manner and responsible for appropriate charge capture in designated system, sending notices and follow up as appropriate.
- Ensure billing and documentation compliance through completion of site WIP/TAD lists, answering provider routine questions, and schedule/hosting/participating in the monthly meeting with Revenue Cycle Management billing team.
- Ensure completion of mandated forms and requests as appropriate to include, but not limited to, death certificates, physician office requests, State specific Workers Compensation and Motor Vehicle Department reporting compliance, and pharmacy requests.
Required Experience and Competencies
- Two to three years of experience in an office or healthcare setting required.
- Associate or Bachelor’s degree in Business Administration, Human Resources or related field strongly preferred.
- Experience working in the healthcare field is preferred.
- Knowledge of healthcare and medical terminology preferred.
- Knowledge of general Human Resource principles preferred.
- Knowledge of Federal, State, and County Agencies who regulate the Healthcare Industry preferred.

Our team approach supports new and experienced nurses. We guide and encourage patients before, during and after their course of treatment. We triage our patients over the phone and in person. We work closely with radiation and surgical oncology, nutrition, and palliative medicine to coordinate care for our patients. Responsibilities
Provides and facilitates competent health care of patients and families in area of practice. The staff Registered Nurse (RN) is a professional caregiver who assumes responsibility for a group of patients for a designated time frame and provides care to these patients via the utilization of the nursing process, nursing trends, technology, financial and human resources within the philosophy of the institution. Responsibilities
Serves as a consultant when difficulties with lactation are beyond the knowledge and/or training of the health care provider. Consults, develops and implements lactation plans of care. Serves as a liaison and/or facilitator between the community and hospital providing ongoing expertise, education and support to groups and individuals. Provides a communication link between the hospital and health care provider's office to assure coordination of patient care and maintains communication with health care providers concerning patients experiencing lactation and breastfeeding problems. Demonstrates appropriate assessment, planning, intervention, and evaluation based on stages of life and developmental status. Educates patients, families and health care team members. Provides education for mothers and family in correct lactation management. Develops and implements ongoing lactation educational program annually on lactation for the healthcare team. Participates in lactation program development and implementation. Works with the appropriate shared governance structure on issues relating to lactation. Participates in development of policies, procedures, algorithms and guidelines specific to lactation practice at site and system levels. Collects data, identifies, monitors and evaluates quality and safety issues related to practice. Maintains phone consultation system for lactation questions and problems. Represents the lactation department at unit/system/community meetings. Evaluates and integrates research findings into lactation practice. Maintains current knowledge of lactation research and reflects this knowledge in current lactation practice. Participates in lactation research projects. The Clinical Resource Unit (Float pool) is committed to our Nursing Professional Practice Model; we believe in caring relationships with our patients, families, community, and colleagues to provide exceptional care.
