Nurse Practitioner for In-home Visits
$2,500 to $4,000 per week

At Signify Health, part of CVS Health®, our mission is to build trusted relationships to make people healthier. We’re hiring board-certified nurse practitioner contractors to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. 

  • Conduct in-home visits that focus on a person’s overall health and well-being and identify potential barriers to care
  • Earn up to $2,500 to $4,000 each week (Average 6-9 visits daily, base rate of $80 per completed assessment)
  • Increase your per-visit rate up to 160% with our Market Multiplier*
  • Earn additional compensation via our pay-per-test model (for visits where the health plan member qualifies to receive additional diagnostic and preventive tests)
  • Shifts available Monday - Sunday (no nights/weekends required)
  • Short-Term or Long-Term basis
  • Pre-scheduled visit in-home w/ health plan members
  • Airfare, Hotel, and Rental Car available (depending on eligibility)

These visits include a medication and medical history review and a physical evaluation performed in a health plan member’s home. Our NP contractors can earn a competitive wage (or supplement their current income) while managing other personal and clinical commitments. You will be provided a pre-populated, patient-specific form for each evaluation and do not prescribe medicine, order lab tests, or alter people’s current treatment regimen.

Summary:

Essential Work Functions:
  • Receive and conduct comprehensive assessments of patients transferring from the operating room and assess their post-anesthesia condition
  • Monitor and evaluate patient’s vital signs, level of consciousness, and recovery from anesthesia
  • Administer prescribed medications and treatments in accordance with approved post-anesthesia nursing techniques
  • Maintain awareness of comfort and safety needs of recovering patients
  • Observe and document patient conditions, treatments, and significant incidents and respond to anesthesia and treatments, notifying the care team of significant changes or adverse reactions
  • Respond promptly to emergency situations utilizing PACU nursing standards, policies, procedures, and protocols
  • Accurately document patient assessments, interventions, and recovery progress in the medical records
  • Develop and deliver individualized patient and family education plans, including detailed discharge instructions and recovery guidelines
  • Collaborate with the surgical team and other healthcare providers to ensure comprehensive post-operative care
  • Participate in patient handoff communications to ensure continuity of care between OR, PACU, and post-PACU units
  • Operate and monitor PACU-specific equipment, such as ventilators, monitors, and infusion pumps

  • 450-bed facility and a primary teaching hospital for the Southern Illinois University School of Medicine.
  • Level I Trauma Center with a 54-bed closed ICU and 22 ED beds.
  • Annual volume of 20,000 with 2-3 admits per day per intensivist and 3-5 admits per night per intensivist.
  • Epic EMR with Dragon dictation.

  • STEMI Receiving Center and Stroke Center.
  • Annual volume of 60,000.
  • Full panel of sub-specialty backup with excellent support.
  • No procedures required; intensivist runs all codes.
  • CT, MRI, and lab available on site.
  • Vituity-staffed Emergency Department, Hospitalist Medicine, and Critical Care offer integrated care solutions at this site.

The Opportunity

  • Seeking Board Eligible/Certified FM, IM, or EM physicians to work "Fast Track" in the Emergency Department.
  • Additionally, ED is staffed by Board Certified EM Physicians at all times.
  • Work along side APs in Fast Track.
  • Monday-Thursday 1pm-11pm.
  • Current IL state license is required.
  • ACLS Certification required.
