- Assure completion of admission or discharges during shift
- Attend rounds (wound, physician etc)
- Provides oversight of LPN assessments (as assigned) and co-signs nursing assessments and initial care plans.
- If acting in the capacity of charge nurse, may develop patient care assignments (appointing other staff, RN, LPN or PCA to implement an established care plan for a designated period of work time), create schedules, and contribute to performance evaluations and educating other staff on HSL policy and procedure.

III. Qualifications:
- Graduate of approved school of nursing required; BSN preferred
- Current Massachusetts license as an R.N. in good standing

The Community Clinical Liaison performs a key role in the generation of referrals to HSL Home and Community Based Services with primary focus on Home Health and Hospice referral generation.

I. Position Summary:The Hebrew SeniorLife Hospice is designed to provide a Jewish Hospice service to terminally ill patients and their families in the greater Boston area. Hebrew SeniorLife Hospice works collaboratively with patients and families in the final months, weeks, and days of life, with the goal of achieving optimum healing of body and soul when curing is no longer an option. As a member of the interdisciplinary group (IDG), the Hospice Registered Nurse provides professional nursing care to the Hospice's patients. Identify patient/family needs and provide supportive care in accordance with the attending physician's orders, the IDG plan of care and the Hospice's policies and procedures. The Hospice Registered Nurse provides care under the direction of the attending physician, the Hospice Medical Director, the Clinical Director, and in compliance with the state's Registered Nurse Practice Act.
II. Position Responsibilities:
- Assumes responsibility for a patient visit or on-call schedule which includes the assessing, planning, implementing and evaluating phases of the nursing process.
- Provides Hospice nursing care based on systematic assessment focusing on pain control and symptom management.
- Initiates communication with attending physicians, other Hospice staff members and other agencies as needed to coordinate care and use of resources for the patient/family.
- Assesses the patient and develops an individualized and comprehensive plan of care in partnership with the Interdisciplinary group and in accordance with Hospice regulations/guidelines. Routinely assesses patients response to plans of care and performs comprehensive assessments, hospice aide supervision, and coordination/oversight of hospice services provided in assisted living, LTC, SNF and hospital settings.

- Obtains data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporates that data into the plan of care.
- Maintains up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur in accordance with agency policy and Medicare regulations.
- Accurately documents assessment findings, observations, interventions and evaluations pertaining to patient care management and services provided in accordance with Hospice policy and procedures on the day services are rendered. Submits documentation in a timely manner.
- Teaches, supervises and counsels patients/families regarding physical care and comfort, and other problems related to the patient's terminal condition.
- Provides guidance to patients/families to assist them in preparing for and coping with anticipated physical and psychological events throughout the dying process. Provides appropriate support at time of death and period of bereavement.
- Meets regularly with Hospice nursing staff to review problems or unique issues from caseloads, share professional support, and exchange feedback aimed toward enhancing professional growth.
- Participates in IDG conference as needed and facilitates discussion of issues from caseload for full staff discussion, consultation and evaluation.

- Shares in providing 24-hour, seven-day a week coverage (evening and weekend on-call) to patients/families.

You will mentor a dynamic team of qualified Educators, Nurses, Occupational Therapists, Physical Therapists, Speech and Language Pathologist, Licensed Social Workers and Mental Health Clinicians as they provide home and community-based care in an environment that’s never the same from one day to the next.
- Provide administrative supervision and support to clinicians to include, but not be limited to, scheduling, document review, referral assistance, policy and procedure compliance, employee relation matters, and EHR assistance
- Assist and/or manage employee recruitment and retention activities to include, but not be limited to interviewing, evaluating, and other employee engagement activities
- Review all incoming referrals for therapy and assign to appropriate clinician based on discipline, specialty, geographic location, etc.
- Provide and/or participate in trainings and consultations as necessary
- Provide program development and marketing in the community
- Assist with Utilization Review if applicable
- Work as a liaison with other South Bay - Mentor Departments regarding referrals, clinical collaboration, trainings, and other matters driving the success of the clinic and the team(s)
- Assist clinic management team in overall functioning of the clinic or team as necessary
- Provide on call duties for emergency consultation/intervention to clients and therapists as needed / if appropriate
- Assist in the research, planning, development, and implementation of clinical and administrative policies

Qualifications:
- Master’s Degree in Social Work, Mental Health Counseling, Clinical Psychology, Counseling Psychology, Early Childhood Education, Special Education or some other related degree.
- Three years of related experience with one year of experience in clinical supervision.

Duties
Registered Nurse provides direct patient care services in accordance with legal, ethical and institution standards. Key Responsibilities
- Conducting AAC related research and/or developing AAC related projects within the scope of this clinical position is encouraged. Clinicians in the ACP are strongly encouraged to develop their own areas of clinical interest related to AAC assessment and intervention that supports the mission of the program.
- Comprehensive evaluation and management of pediatric communication disorders that reflect an in-depth understanding of a child’s speech, language, and communication profile and needs.
- Providing parent/caregiver education and counseling regarding diagnoses and follow-up services and referrals.
