Job Description 

Works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services across the continuum of care. Maintains ethical principles and professional standards of practice in adherence to the discipline’s professional code of ethics. 

•    Performs clinical diagnosis in conjunction with conducting a comprehensive bio-psychosocial assessment and establishment of a care plan.

Job Summary

Supervisor of Behavioral Health works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services across the continuum of care for Adults and Child/Adolescents. Key areas of responsibility: 

Unit and Personnel Management

• Supervise all clinical care, including use of behavioral health interventions, emergency management and monitors compliance with requirements for treatment planning and transition planning.

• Assesses staff performance and develops clinical team.
• Identifies opportunities for operational improvement and plans to implement them in collaboration with Program Manager.

Direct Patient Care

• When required, conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate.
• Provides direct patient care that may include: case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end-of-life care, referral and discharge planning.

Clinical Excellence

• Leads administrative and development activities including but not limited to: team meetings, clinical supervision of staff and staff education.
• Provide field supervision to second year student interns

• Master's degree in social work, counseling or marriage & family therapy required.
• Current Connecticut license; LCSW, LPC, or LMFT required.
• 1 year of management experience in a healthcare related setting required. 
• 2 years of experience as a licensed Clinician required.
• Previous experience in clinical setting with Adults or Children/Adolescents populations
• Computer proficient
• Ability to assess problems, determine solutions and make decisions within a timely manner
• Utilize supervision and team approaches in the context of patient assessment and clinical interventions

Be the Best Hospice Admissions RN You Can Be

If you meet these qualifications, we would love to meet you:

• Licensed as a Registered Nurse in the state(s) of practice, with an associate degree preferred and CPR certification recommended (required for direct patient care in Georgia). Must meet state-specific regulatory requirements.
• Previous nursing experience in hospice, home health, general medical-surgical, or population-specific care (e.g., pediatrics, geriatrics) is required, along with willingness to serve on-call.
• Must have a reliable vehicle, valid driver’s license, and the minimum required liability auto insurance.
• Strong communication and organizational skills, with working knowledge of Microsoft Office Suite and EMR systems. Must be able to work effectively as part of an interdisciplinary team.
