The Certified Case Manager (CCM) serves as a key member of the interdisciplinary team and actively manages and directs resource utilization to achieve the highest quality outcomes during a patient's rehabilitation experience.

The CCM coordinates and advocates for the patient during their hospitalization and from admission to post discharge.

As an effective communicator, the CCM manages information to effectively oversee health care delivery and facilitate interdisciplinary plan of care decisions.

The CCM facilitates timely communication regarding the patient's care, establishes and monitors the discharge plan implementation while identifying and addressing patient's psychosocial and support systems issues.

The CCM oversees the effective coordination of services and manages issues in the following main areas: admission and discharge, team conference and interdisciplinary plan of care communication, patient and family education, payor relations and total fiscal management.

The CCM performs ongoing utilization review and acts as a liaison to the payor while assuring that cost effective treatment is provided by the team.

The CCM assures that regulations regarding patient's rights are fulfilled.

We’re hiring board-certified nurse practitioner contractors to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. 

• Conduct in-home visits that focus on a person’s overall health and well-being and identify potential barriers to care
• Earn up to $2,500 to $4,000 each week (Average 6-9 visits daily, base rate of $80 per completed assessment)
• Increase your per-visit rate up to 135% with our Market Multiplier*
• Earn additional compensation via our pay-per-test model (for visits where the health plan member qualifies to receive additional diagnostic and preventive tests)

These visits include a medication and medical history review and a physical evaluation performed in a health plan member’s home. Our NP contractors can earn a competitive wage (or supplement their current income) while managing other personal and clinical commitments. You will be provided a pre-populated, patient-specific form for each evaluation and do not prescribe medicine, order lab tests, or alter people’s current treatment regimen.

We're seeking a fulltime NP with a minimum of two or more of experience to provide fulltime outpatient care.
