We are seeking an experienced RN leader for a Director of Case Management opportunity at a large, high-volume acute care hospital in the Bradenton, FL area.
This is an excellent opportunity for a strategic, results-driven leader to oversee a well-established Case Management and Social Work department within a 383-bed Level II Trauma facility.
Position Overview
The Director of Case Management is responsible for overall leadership, coordination, and operational performance of the department. This executive-level leader ensures compliance with hospital policies, regulatory agencies, and industry standards while optimizing patient care coordination and discharge planning processes.
This individual partners closely with executive leadership, including the Division Director of Case Management and CFO, and serves as a key driver for Length of Stay (LOS) management, care coordination excellence, and clinical resource utilization.
Key Responsibilities
- Direct and evaluate all case management department operations
- Oversee staffing models, skill mix, FTE allocation, onboarding, and competencies
- Ensure compliance with regulatory standards and organizational policies
- Lead care coordination and discharge planning processes
- Improve LOS performance and optimize resource utilization
- Analyze operational metrics and implement sustainable improvement initiatives

Job Summary and Qualifications
